SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP)
FINAL REPORT NOS. 50-245/92-99,50-336/92-99 AND 50-423/92-99
This letter fervards the final Millstone Nuclear Power Station SALP Report (Enclosure 1)
for the period between February 16,1992, and April 3,1993, for Millstone Units 1,2 and
3. The initial SALP report was forwarded to you by our July 20,1993, letter (Enclosure 2)
and was discussed with you and other members of your company at a meeting on August 24,
1993, at your training center at the Millstone site.
We have reviewed your detailed written response dated September 10,1993 (Enclosure 3).
We will continue to closely monitor your efforts to improve corrective action effectiveness
and compliance with procedures as well as to correct the operator traming weaknesses
observed at Millstone Unit No.1. In addition, your recent performance problems at
Millstone Unit 2, which have been the subject of several discussions with the NRC and
resulted in an enforcement conference on October 1,1993, on the apparent violations related
to the repair of letdown isolation valve 2-CH-442, highlight the need for continued and
sustained performance enhancement at Millstone Station.
Based on your written comments, we determined that two clarifications and corrections to the
initial SALP report were warranted and these have been incorporated into the final report.
The first change involved a clarification on page 21, paragraph 4 in the emergency
preparedness section regarding the motives behind the recent substantial staff increases to
your emergency preparedness staff. We found that our previous discussion did not fully.
reflect your primary goal of enhancing on-site performance. The second change involved
correcting a typographical error on page 29, paragraph 3 as well as clarifying a subsequent
statement in that paragraph which could be misinterpreted. The final SALP report has been
revised to reflect these changes as indicated in Enclosure 4.
