Annual Radioactive Effluent Release Roport in accordance with I'echnical Specifications 3.3.3.11.b for Unit 1 and 3.3.3.10.b for Unit 2.
c A Station Design Change was performed during which the ventilation vent stack l
flow rate monitors where inoperable. These monitors were tagged out March 22,1989 for modifications to electrically isolate the ventilation stack flow signals for the' Kaman Science Radiation Monitoring System.

Report were unaware of the inoperable status of the flow rate monitors at the time. For preparation of past reports, Station Deviation Report logs would be reviewed ~to determine if instrumentation was cut of service and needed to be reported. Beccuse a-Design Change caused the ineperable status, a Deviation Report was not prepared.
3 Therefore, individuals preparing the Semi-Annual report did not include the ventilation _
vent stack flow rate monitors in the inoperable instrumen'ation section of the report. To prevent a recurrence of this problem, we are developing procedures to ensure that Health Physics will know when a monitor required by Technical Specifications is out of service for an extended period.
