VERMONT YANKEE SURVEILLANCE INSPECTION REPORT NO.

50-271/93-31

This letter transmits the findings of the special safety inspection conducted by

Mr. S. L. Hansell of this ofHee from November 15 through November 15:,1993, at the

Vermont Yankee Nuclear Power Plant, Vernon, Vermont. The inspection was a followup 'to

the Operational Safety Team inspection (OSTI) conducted between June 7 and July 2,1993.

At the conclusion of the inspection, Mr. Hansell held discussions of his findings with

members of your staff.

The inspection focused on areas related to safe operation of the plant and the OSTI team's

noted weaknesses related to the surveillance program. The primary objective was to

determine whether the surveillance of safety-related systems and components was conducted

in accordance with approved plant procedures and technical speciHcations. Also, open items

related to the reactor recirculation jet pump technical specincation surveillance, removal of

high pressure feedwater heaters from service, and chemistry technician training qualification

were reviewed.

Overall, the surveillance test program was implemented in accordance with approved plant

procedures and technical specifications applicable to this inspection. The recent program

upgrades adequately addreseed all of the OSTI's noted program weaknesses. However, a

new weakness was noted for the tracking of safety-related pumps that required an increased

frequency surveillance test. Plant personnel used an informal document to ensure the proper

surveillance frequency was performed instead of an existing procedure change form to update

the computer automatic tracking system. You are requested to provide a written response to

the concerns identined in section 3.2.2 of the enclosed report within sixty days of receipt of'

this letter.

The corrective actions associated with the four open items referenced above were reviewed

and found to be acceptable (sections 4.1,4.2,4.3, and 4.4). - Within the scope of this

inspection, no violations were identified.
