SUBJECT:

CONCERNS WITH NRC HANDLING OF ALLEGATIONS AT MILLSTONE On June 6,1991, the NRC Resident Inspection Office received an allegation from a licensee employee concerning recurring problems with tagging of equipment at the Millstone station.

In addition to the technical concerns, the alleger also stated that the NRC was aware of the concerns for almost 4 years and has failed to take positive action to correct the problem. A copy of the Allegation Receipt Report, as well as the alleger's June 3,1991 memorandum, is enclosed.

I am providing this information to you for whatever action you deem is appropriate. The identity of the alleger should be protected consistent with the requirements set forth in NRC Manual Chapter 0517 and its appendices.

I note that NRC Region I has, in the past, looked at and evaluated each equipment tagging issue brought to our attention on its individual merits. Region I has identified only one potentially safety signi6 cant tagging error to date, and that involved an error which rendered a Unit 3 Diesel Generator inoperable. While other concerns involving lack of attention to detail in equipment tagging have been identified, they have been less significant in nature.

The Region has identified problems with attention to detail in several other areas, most notably in the areas of drawing control, procedural compliance, and operator errors. The tagging errors represent another symptom of the bigger issue which is an NRC concern that was specifically mentioned in the most recent Millstone SALP. This issue was also a topic o discussion at the recent Senior Management Meeting held in Region III.

another site and 4 contractor specialists, to aid in the resolution of allegations at that station.

The Task Force will be reviewing and evaluating safety tagging of equipment at the Millstone station in more detail. The Resident Inspectors will assist in this effort.
