e ENCLOSURE VOGTLE ELECTRIC GENERATING PLANT-UNITS 1 & 2 REPLY TO A NOTICE OF VIOLATION NRC INSPECTION REPORTS 50-424:425/93-20 The following is a transcription of the violation as cited in the Notice of Violation (NOV):

" Technical Specification (TS) 6.7.1.a. requires, in part, that written procedures be established, implemented, and maintained covering the activities in Appendix A of -

Regulatory Guide 1.33, Revision 2 (Rev. 2), dated February 1978.

Regulatory Guide 1.33, Appendix A, Quality Assurance Program Requirements, Rev. 2, dated February 1978, lists, in part, procedures required for operation of safety related systems including atmospheric cleanup systems.

Licensee Procedure 54049-C, Safety Related Carbon Absorber Operation Time Log, Rev.

2, dated March 17,1992, requires, in part, that upon notification from Operations or other sources, a responsible individual is to record the sequential number, brief description of reason for start, start and stop times, total and cumulative run times, and the reading.

recorded at run time indicator for the applicable unit, if provided, on the log sheet for the Fuel Handling Building (FHB) engineered safety feature (ESF) ventilation units.

Significant discrepancies between calculated run times and run time indicator are to be investigated and appropriate values are to be used for TS sampling purposes.

Contrary to the above, from March 25,1992 through September 1,1993, the licensee -

f failed to log the operational data and investigate significant discrepancies in run times for approximately 50 and 60 operational events for Trains A and B, respectively, of the FHB ESF ventilation atmospheric cleanup system.

i This is a Severity Level IV violation (Supplement I)."

Admission or Denial of the Violation This violation occurred as stated in the NOV.

Reason for the Violation j

i This violation was attributed to the failure of the responsible engineer (RE) to follow procedural guidance that delineates logging requirements for carbon absorber operation. The RE relied primarly on run-time meters for determining if tests of affected charcoal absorbers were required instead of completing run-time logs as required by Procedure 54049-C. Subsequent to this inspection, an additional review was performed, and it was determined that no carbon absorbers had exceeded the 720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br />.

VOGTLE ELECTRIC GENERATING PLANT-UNITS I & 2 REPLY TO A NOTICE OF VIOLATION NRC INSPECTION REPORTS 50-424:425/93-20 Corrective Steps Which Have Been Taken and the Results Achieved The carbon absorber run-time logs were updated to reflect any operation and reviewed to ensure that Technical Specification (TS) requirements were met. The review determined that no TS requirements had been violated. Additionally, appropriate personnel have been disciplined regarding the imponance of adherence to procedure and timely logging / review of carbon absorber operation.

Reviews were also performed on other carbon absorber run-time logs to determine if this was an isolated case of one individual failing to follow procedure. The review concluded that this was an isolated case, and no other logs were found to be in a similar condition.

Corrective Steps Which Will Be Taken to Avoid Further Violations All corrective actions have been completed and no further action is warranted at this time.

Date When Full Compliance Will Be Achieved Full compliance was achieved on September 8,1993, aller the appropriate logs were updated and reviewed to verify that no carbon absorbers had exceeded the 720 hour0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br /> run-time without the required testing as a result of the inadequate log keeping.
