Attachment to WO 93-0190 Page 1 of 6 SPECIAL REPORT 93-003 FIFTH STEAM GENERATOR TUBE INSPECTION RESULTS This report is being submitted pursuant to Technical Specification 4.4.5.5.b and 6.9.2 to report the results of the fifth steam generator tube inservice inspection.

The fifth-steam generator tube inservice inspection was completed by Westinghouse Electric Corporation on March 31,

1993, during the ' sixth refueling outage.

A total of 5623 tubes were inspected and a total of nine tubes were plugged in steam generator "B"

as discussed-in Special Report 93-002, which was transmitted by letter WO 93-0072 dated April 7, 1993. ' Included in this inspection effort were tubes which had been previously identified as having indications requiring re-inspection during this inservice inspection effort.

The inspection results, which includes the location and percent of-wall-thickness penetration for each indication of.an imperfection,'are listed in Section 3 of this report.

During the preparation of this report, discrepancier were discovered in Special Reports90-003 (Letter NO 91-0111, dated April 3,

1991) and 91-007 (Letter NO 92-0130, dated May 29, 1992) which do unented the results of Wolf Creek's third and fourth. steam generator t _th e _ inservice inspections.

Specifically, only the location and percent of w Gl-thickness of indications which - were greater than or equal to 20 percent of the nominal tube wall thickness were reported.

This is contrary to the requirement to include the location and percent of wall-thickness penetratio.1. for, all indications of

' imperfections.

Manager, it was decided that Special Reports90-003 and 91-007 would not be revised at this time.

NRC Bulletin 89-01, " Failure of Westinghouse Steam Generato r Tube Mechanical Plugs," with supplements, requires that mechanical tube plugs manufactured from Inconel 600 with certain heat numbers be removed prior to the expiration of their predicted time to fail.

As a conservative ' measure,.. all of the Inconel 600 mechanical tube plugs were removed from steam generator "B" during this inspection.

In addition, all Inconel 600 mechanical tube plugs are planned to be removed from steam generator "D"

during ; the seventh refueling outage scheduled for Fall, 1994.

Three Inconel 600 mechanical tube plugs (1 hot leg and 2 cold leg) were removed from two tubes in steam generator "B*.

All of the tubes were replugged.

All newly installed mechanical plugs were -manufactured from Inconel 690.

A total of seventeen plugs were installed in steam generator "B".

Ten plugs--

were installed based on eddy current findings, three plugs were installed.as'a.

result of plug removal, and four f actory installed 26" stub tube plugs-were mechanically plugged.
