NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS.50-334/93-26

AND 50-412/93-28)

This refers to the inspection conducted by Messrs. L. Rossbach, P. Sena, and S. Greenlee of

this office from October 26 to November 27,1993. The inspection included a review of

activities authorized for the Beaver Valley Power Station in Shippingport, Pennsylvania. At

the conclusion of the inspection, the findings were discussed with Messers. D. Spoerry,

'

G. Thomas, and other members of your staff.

Areas examined during the inspection are identified in the report. The inspection consisted

of document reviews, interviews, and observation of activities important to public health and

safety. The purpose of the inspection was to determine whether activities authorized by the

license were conducted safely and in accordance with NRC requirements.

Based on the results of this inspection, certain of your activities appeared to be in violation

of NRC reqairements, as specified in the enclosed Notice of Violation (Notice). The first

violation involves multiple examples of procedure violations by operators. All but one cf

these examples were self-identified by your staff, and taken individual'.y, each is of minor

safety significance. Nevertheless, this violation is being cited because the presence of

multiple examples and previous cittd and non-cited procedural violations indicates the need

for additional corrective actions. The second violation involves the failure of an instrument

due to inadequate installation specifications. This is of concern because your design controls

should ensure that qualification test conditions are properly translated into installation

specifications.

Also, a non-cited violation was identified for not developing several alarm response

procedures. This violation is not being cited because of its minor safety significance and

your corrective actions.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. In your response, you should document the

specific actions taken and any additional actions you plan to prevent recurrence. After

reviewing your response to this Notice, including your proposed corrective actions and the

results of future inspections, the NRC will determine whether further NRC enforcement

action is necessary to ensure compliance with NRC regulatory requirements.
