SERVICE WATER SYSTEM OPERATIONAL PERFORMANCE INSPECTION
(50-331/93018(DRS))
This refers to the reduced scope service water system operational performance
inspection (SWSOPI) conducted by Mr. Rolf A. Westberg and other NRC
representatives of this office on October 25 through November 5, 1993. The
,
inspection scope was reduced because of the results of tha 1990 NRC safety
'
system functional inspection (SSFI) of your emergency r,ervice water system.
!
The inspection included a review of authorized activities int your Duane
Arnold Energy Center. At the conclusion of the inspection, the findings were
discussed with those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. The team
!
assessed the service water system's (SWS) operational performance.
In
particular, the team reviewed the SWS's design, maintenance, operation,
i
surveillance and testing. The team also assessed the planned or :ompleted
actions for Duana Arnold in response to Generic Letter (GL) 89-13, " Service
Water System Problems Af fer. ting Safety Related Equipment," issued on July 18,
1989.

The team considered Duane Arnold's SWS design and operation satisf actory.
During the inspection, the team identified a number of program strengths.
For
.
example, experienced and technically competent mechanical engineering staf f,
good plant material condition, incorporation of SWS changes into the operator
,
'
training program, and simulator use by Engineering to evaluate SWS changes
prior to implementation. The team also considered your SWS self-assessment to
be a good effort as it identified many of the same issues as our team.
While the team did not identify any SWS equipment that would be unable to

perform its intended safety function, some program weaknesses were identified.

The most significant was the incomplete implementation of the heat exchanger

performance monitoring program. Other weaknesses included incorrect computer
modeling of SWS components in the simulator and lack of testing of check
.,
'
valves which isolate well water from emergency service water (ESW) at the
control room chillers.
!
No violations of NRC requirements were identified during the course of this
inspection.
One unresolved item (331/93018-01) was identified regarding a
well water line in the high pressure coolant injection pump room that was not
reviewed under the original IEB 79-14 program. We request that you submit
within 60 days of the date of this letter, a written response detailing: (1)
'
the steps that will be taken to address this issue; and (2) the date when your
'
analyses will be completed.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosed inspection report will be placed in the NRC
Public Document Room.
