The the reason for failure to label the radioactive exposure device was l

not complyin6 funy with 10CFR34.20(b)(1)(v).

f The corrective steps taken was to fasten metal bands containin6 the t

companies name, address, and telephone number to each of the exposure devices.

A form has been set up in our files <,nowin6 weekly inspections' of said I.D. Innds noting wear conditions for promptin6 replacement.
