requires submittal of a.Special Report if the fire suppression water system is inoperable. This report describes an event in which it was found that-both fire pumps were inoperable for a period of 15 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br /> and 19 minutes.

At 0721 on July. 20, 1990, while conducting daily rounds, a plant operator found the circuit breaker which supplies the electric motor-driven fire pump in the open position. Although the electric motor-driven fire pump could.have been manually started, it would have been unable to automatically start with the breaker in the open position.- The diesel-driven fire pump was out of service for annual maintenance at the time'the open breaker was discovered. The breaker was promptly reset and the electric motor-driven fire pump was returned to an operable status.

During'a review of the fire protection-system alarm printer, it was i

determined.that the electric motor-driven fire pump had automatically started at.1602 on July 19, 1990.

The cause of the automatic start was low pressure in the fire water syJtem which occurred while filling a sprinkler system which had been temoved from service for maintenance.

The Assistant Control Operator secured the electric motor-driven fire

-pump following the automatic start but did not reset the circuit breaker until it was discovered open on July 20, 1990.

The Assistant Control Operator was counseled.regarding the need for L.

attention to detail while conducting work activities.

Although the Assistant Control Operator knew the requirement to rese'. the circuit breaker, Operating Instruction 5-4, Fire Protection, will be revised to include explicit instructions for securing the electric motor-driven fire pump.

There was a fire pumper truck onsite and connected to a supply of water-during the period the fire pumps were inoperable.

The truck could have supplied fire suppression water if it had been required.

The truck had been brought onsite as a backup fire suppression system to support the diesel-driven fire pump annual maintenance activities.

This event was also reported via telephone and' facsimile transmission in accordance with Trojan Technical Specification 3.7.8.1 on July 20,-1990.
