INP0/NUMARC/NRC C0 ORDINATION MEETING MINUTES JULY 16, 1990 On Monday, July 16, 1990, the staff of the Division of Licensee Performance and Quality Evaluation met with representatives from INP0's Division of Training and Education and NUMARC's Operations, Management and Support Services Division to exchange information about requalification training in the nuclear industry. The meeting was held in accordance with the Memorandum of Agreement, dated October 20, 1988.

SUMMARY
members of the Division of Licensee Performance and Quality 16,('1990,-DLPQ) staff met with the Institute of Nuclear Power O On July (INPO) and the Nuclear Management and Resources Council (NUMARC) perations Evaluation at the NRC office in Rockville, Maryland. The purpose of the meeting was to provide a forum for the exchange of information as described in the INP0/NRC Memorandum of Agreement dated October 20, 1988. A list of the meeting attendees is provided as Attachment I to this report. The following topics were discussed:
1.
Generic problems identified during the licensed operator requalification program evaluations.
The NRC updated INPO and HUMARC on the generic, recurrent findings identified during the 79 requalification program evaluations completed to date. The Safety / Technical and Administrative / Program findings discussed during the meeting are identified on an Attachment to the meeting agenda. Most of the deficiencies occurred at the ten facilities that received overall grades of unsatisfactory, but some were also applicable to facility programs that were evaluated as satisfactory.
The NRC expressed concern that many of these generic issues were not being adequately communicated to the industry and indicated its intent to summarize them in an Information Notice addressed to all facility licensees. The NRC believes that if facilities knew where to focus their efforts they could enhance plant s.fety more promptly, avoid the pitfalls of their predecessors, and minimize the likelihood of an unsatisfactory requalification program evaluation.
The intermediary roles of INP0 and NUMARC in facilitating communications between the NRC and the nuclear industry were also discussed. The NRC indicated that it was very receptive to feedback on the requalification examination process from the industry and individual operators.
It pointed out that many of the changes made to ES-601 in Revision 6 of the Operator Licensing Examiner Standards were as a direct result of industry feedback provided at a meeting hosted by NUMARC in Dallas, Texas in February 1990.
Representatives from NUMARC were very positive about the success of the Dallas meeting, and they plan to host similar gatherings on an annual basis. All parties expressed an interest in continuing the dialog on training and examination issues in that and other working group meetings.

INPO methods of measuring crew performance and training techniques in the simulator.
Representatives from INP0 briefed the NRC staff on the methodology used to assess crew performance and training techniques during INP0 plant evaluations. The discussions focused on the program's goals, team composition and qualifications, team preparation, scenario selection, evaluation methodologies, and results/ findings.

Accredited training programs being declared unsatisfactory based on NRC administered examinations.
Representatives from INPO and NUMARC agreed with the NRC's belief that the requalification examination failure rates were high. The NRC expressed concern over the fact that it is finding some accredited licensed operator requalification training programs which do not meet some of the minimum standards established in the Examiner Standards.
]NPO informed the NRC that it had received feedback from some facilities that the requalification examination methodology described in the Examiner Standards (ES-601) was not being consistently implemented. The NRC requested specific information to address the aroblem.
