Enclosed is the Systematic Asse sment of Licensee Performance (SALP) Report

for the Waterford Steam Electric Station, Unit 3, covering the period

August 2, 1992, threugh October 30, 1993. We have scheduled a meeting to

discuss this repn

with you and your staff at the Waterford 3 facility

training center on December 21, 1993, at 3 p.m. (CST).

During this meeting

,

you are encouraged to candidly discuss any comments you may have regarding our

report. While ints meeting is considered a presentation and discussion forum

between Entargy Operations, Inc., and the NRC, the meeting will be open to any

other interested parties as observers.

Overall you continued to operate the plant safely and, it is my view, that

your conduct of activities in connection with the Waterford 3 facility was

generally strong. The basis for this performance includes your experienced,

.

stable, and well-trained staff; excellent plant material' condition; and strong

corporate support for program improvements, including peer-group evaluations.

These attributes are tempered by instances of inattention to detail, a lack of

self-checking, and corrective action system weaknesses. We noted a decline in-

management presence and visibility in the plant spaces.

These issues, as well

!

as an increase in operating events, resulted in a decline in performance in

!

the plant operations area. We noted superior performance in the areas of

maintenance and plant support. Your staff is commended for their aggressive

approach to plant aging issues, radiological control practices, and security

program implementation.

Items noted late in the SALP period involving

emergency preparedness exercise weaknesses and isolated problems with

procedural adequacy and procedure' implementation in the radiological controls

area warrant attention to ensure that superior performance is maintained in

the plant support area.

In the engineering area performance was mixed. Continued initiatives to

improve the organization by consolidation in a new, onsite facility and

streamlining the management structure were noted. These and other strengths,

such is systems training for senior managers, and a strong technical response

to identified issues' were considered superior. Engineering effectiveness at

Waterford 3 continues to mature, and a period of sustained performance

demonstrating the proactive identification of issues, including challenging

operational safety decisions, is needed to support an overall superior

performance rating.

Your attention is directed towards these challenges in the operations and

engineering areas as well as providing for an appropriate level of management

presence and oversight in the plant. These considerations are appropriate to

support the continuing identification and resolution of. issues and direct

confirmation of the effectiveness of your improvement initiatives.
