This refers to the ins)ection conducted by Messrs. W. F. Smith, S. D. Butler,

and W. M. McNeill of t11s office during the period October 2 through November 13,

1990. The inspection included a review of activities authorized for your -

Waterford Steam Electric Station, Unit 3, facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

-identified in the enc'osed report.

' Areas examined during the inspection are identified in the report. The areas

included onsite followup of events, monthly maintenance observation, monthly

surveillance ~ observation, operational safety verification, followup of

previously, identified items, licensee event report followup, balance of plant

11nspection,' and evaluation of licensee quality assurance program implementation.

.'Within these areas, the inspection consisted of selective examination of

procedures and representative records, interviews with personnel,' and

e observation of the activities in progress.

-Based 'on-the results of this inspection, certain of your activities appeared to

,, be in violation of NRC: requirements, as specified in the enclosed Notice of

Violation-(Notice). We are concerned about the violation because we have noted

M that the last three monthly resident inspection reports have identified other

failures to follow procedures. Although the consequences have been of minor:

i safety.signif'(cance,1 the failure to follow procedures may be. indicative of

l potential weaknesses in your program for procedure compliance. Procedure

' compliance had also been 'an issue in the 1988-1989 timeframe. Based on

discussions with the resident inspectors, we understand you have recently

s

'

c3nducted assessments of performance in the maintenance area and have also

concluded that the procedure compliance issue should be reassessed. We are

encouraged with the knowledge that you had taken these initiatives. ,

You are .regiaired to respond to thJs ' letter and should follow the instruc'tlons-

specified in the enclosed Notice when preparing 30ur response. 'In your -

' response, you should document the-specific: actions taken and any; additional '

.V . e actions;you plan to prevent recurrence. After reviewing your response to this
