Technical Specification 6.8.1.a states, in part, that
written procedures shall be established, implemented, and

maintained including the applicable procedures recommended

in Appendix A of Regulatory Guide 1.33, Revision 2,

February 197 8.
Item 9.a of Appendix A states, in part,
!
" Maintenance.that can affect the performance of safety-
l
related equipment should be properly preplanned and
performed in accordance with written procedures,
l
documented instructions, or drawings appropriate to the
circumstances."
A note to Step 6.9.2.A of Operations
Engineering Procedure OEP 6.03Q, Revision 7,
" Design
Document Change Control,"
implements.this requirement for
modification work and states, " Items tagged with ECNP/ MOD
,

Hold tag shall not be installed until a ECNP/ MOD package
'
is issued authorizing the use of a given part."
Contrary to the above, on or about June 7,
1993,
i
maintenance personnel installed a replacement reverse
,
power relay, which had been tagged with an engineering
i
change notice / modification package (ECNP/ MOD) hold tag, in
j

the Standby Diesel Generator 23 control panel, without an
ECNP/ MOD package issued authorizing its use.
This is a Severity Level IV violation.

attached to the relay. improperly responded to maintenance

that the associated Document Change Notice (DCN).was
'
sufficient authorization to proceed with installation.

The cause of the improper response was a lack of training

of the on-shift engineer who was contacted.

HL&P also determined during the investigation of this
violation that a sepatale problem existed in that'the
applicable vendor wiring diagram (contained in the SDG
Generator Control Panel Instruction Manual) did not match
the original reverse power relay configuration.
The
vendor wiring diagram showed the relays wired for A-B-C
phase rotation, but the relays had been internally
modified to accommodate the C-B-A rotation. 'This
situation caused the reverse power trips which occurred
during post maintenance testing of Diesel Generator 23.
The most likely cause was a failure on the part.of the
equipment supplier to properly document the internal
modification to the relay.
This problem is not reportable
under 10 CFR Part 21.
IV.
Corrective Actions:
The corrective actions listed below address the violation,
the separately identified internal wiring problem, and the
overall " Engineering Hold" program enhancements.
1.
Appropriate modification documentation has been
completed for the reverse power relay in Standby
Diesel Generator 23.
2.
The engineers currently on-shift have been trained on
the programmatic aspects of the ECNP/ MOD' Hold Tag
program.
The engineers who are not currently on-
shift will be trained upon their return to shift
duties.
The remaining engineers will be trained by-
December 10, 1993.
3.
The En-Tronic Control System Instruction Manual
(4041/8041-01009-CEO) for the Standby Diesel
Generator Control Panels has been amended to correct
the wiring diagram for the reverse power relays.
4.
A training bulletin has been issued to appropriate
pcrsonnel in the Maintenance Department to explain
and emphasize the importance of the Engineering
Notification and ECNP/ MOD Hold. Tagging program.
This
bulletin also emphasized the importance of
maintaining a questioning attitude when performing
work.
5.
Engineering will review the electrical power
distribution systems at STP to determine if there are
other negative phase sequence relays which may be
affected.
This action will be completed prior to
Mode 4 on either unit.

developed to enhance.the overall engineering

notification and engineering hold program, improve
the plant personnel understanding of the process and

provide timely support to Operations and Maintenance.

During the testing of the replacement relays, improper.

polarity (due to the difference in phase rotation between

the originally installed relay and that assumed by the

test procedure, which was based upon the vendor wiring

diagram.) was noted by the technician, but not identified

as a problem.
The calibration procedure applicable to the
reverse power relay has been revised to account for the
proper polarity.
As stated in the corrective-actions

the importance of maintaining a questioning attitude.
