Interim Report of 10CFR50.55(e) on Reactor Mode Switch Failure (#73)
Dear Mr. Keppler:
On July 1,1982, Detroit Edison's Mr. H.A. Walker, Supervisor-Construction Quality Assurance, telephoned Mr. H. Wescott of NRC Region III-to report on a problem with reactor mode switch failure at the Fermi 2 site.
The switch is frozen in one position, and cannot be placed in other modo selections. In addition, scme of the contacts would not maintain their position during operation. For example, a contact "normally open" for a made selection would be open, and when the switch was moved to another pusition and returned the contacts would remain closed. The switch had failed on a previous occasion and had been repaired by General Electric.
This could be a problem with~the design of the switch. _An investigation of the problem'is continuing.
