V0GTLE ELECTRIC GENERATING PLANT - UNITS 1 & 2 NRC NOTICE OF VIOLATIONS 50-424/90-20-01 AND 50-425/90-20-01 AND GPC RESPONSE

" Technical Specification 6.7.la requires that written procedures be establishni, implemented, and maintained covering activities delineated in Appendix A of Regulatory Guide 1.33. Revision 2, February 1978.

Regulatory Guide 1.33, Revision 2, February 1978, delineates the types of safety-related activities that should be covered by written procedures and includes procedures for startup, operation, shutdown, and procedures for performing maintenance.

Operations Procedure 13145-2, Diesel Generators, Revision 10, Section 4.4.2, Emergency Stopping Train A(B) Diesel Generator, provides the steps for securing the diesel generators from an emergency start.

Contrary to the above, Procedure 13145-2, Diesel Generators, was inadequate, in that,it did not provide adequate instructions to shutdown the diesel from an emergency start resulting in an inadvertent restart of diesel generator 2B on September 15,-1990.

This is a Severity Level IV violation (Supplement I)."

RESPONSE TO VIOLATION 50-424/90-20-01 AND 50-425/90-20-01 Admission or Denial of the Violation:

-The violation : occurred as stated in the notice of violation, except for the date cited for'the violation. The date should be September 25, 1990, not September-15, as denoted on page 6 of the inspection report.

Reason for the Violation:

n The reason for the violation was procedural inadequacy.

Procedure 13145-2,

" Diesel Generator," did not provide sufficient directions or guidance on actions for shutting down the diesels after emergency starts.

Corrective"Steos Which Have Been Taken and the Results Achieved:

a After this event, on-shift operations personnel were briefed regarding the

. correct methods for shutting down a diesel generator after an emergency start, n

o A' temporary change of procedure 13145-2 was issued on-September 26, 1990.

Additional training was incorporated into the licensed operator requalification

. training program to provide guidance on actions-to take for shutting down the diesels after emergency starts.

NRC NOTICE OF VIOLATIONS 50-424/90-20-01 AND 50-425/90-20-01 AND GPC RESPONSE Plant procedures for both Units,13145-'l and 2, " Diesel Generators," were revised'and approved on October 9, 1990 to provide guidance on actions to take concerning shutting down the diesels after emargency starts.

Corrective Steos Which Will Be Taken to Avoid Further Violations:

I The corrective steps which have been taken are adequate to minimize the potential for recurrence. No further corrective steps are warranted.
