APPEtolX A NOTI OF VIOLATION Florida Power & Light Docket No. 50-335 St Lucie 1 License No. DPR-67 As a result of the inspection conducted on April 11 - May 14,1982, and in accordance with the PFC Enforcanent Policy, 47 FR 9987 (March 9,1982), the following violation was identified.
Technical Specification 6.8.1 requires that written procedures shall be inplemented. Adninistrative Procedure 0010123, Mninistrative Control of Vaives, Locks and Swtiches, specifies that certain valves irrportant to safety shall be locked in their proper position.
Contrary to the above, during a walkdown of the contairnent spray systan on May 29, 1982, the inspector discovered a valve, V-7166, with its lock and chain hanging on the valve body and not locked through the handwheel.
Mninistrative Procedure 0010123 requires this valve to be locked closed during existing plant conditions. This valve had been repositioned the previous day to provide a drain path for repdiring a leak on an adjacent valve on Equipnent Clearance Order 1-4-139.
Operating Procedure 0010122, in-Plant Equipment Orders, covering that activity faiIed to specify e
during equipnent restoration that locked valves be confinned locked.
This is a Severity Level V Violation (Supplement 1.).
1 Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statenent or explanation in reply, including:
(1) adnission or denial of the alleged viola-tion;- (2) the reasons for the violation i f adnitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full conpliance will be achieved. Consideration nay be given to extending your response time for good cause shown.
