Subject: NRC Inspection Report No. 50-309/90 19 (9/19/90 10/31/90)
This transmits the report of the above subject inspection by Messrs. C. Marschall,
R. Freudenberger, T. Koshy, and Ms. M. Banerjee at the Maine Yankee Atomic Power Plant,
Wiscasset, Maine.
Those findings have been discussed with Messrs. R. Blackmore and
R. Prouty of your staff.
As documented in the enclosed report, several apparent violations are being considered for
escalated enforcement action in accordance with the * General Statement of Policy and Procedure
for NRC Enforcement Actions" (Enforcement Policy),10 CFR Part 2, Appendix C. Two of
these involve a lack of Containment isolat;on Vive position indication limit switch
environmental qualification (EQ) from October 1985 to f\\tober 1990 and failure to promptly
correct this EQ deficiency when identified in Februarf 1990. Two others involve the failure to
provide qualified power supplies for the Containment Isolation Valve position indication system
from October 1985 to October 1990 and the failure to provide accurate associated information
to the NRC in March 1985.
. An enforcement conference to discuss these matters has been scheduled for November 15,1990
in the NRC Region I office. At that conference, please be prepared to discuss the apparent
violations, tlicir causes and safety significance; any corrections to the inspection report,
corrective actions taken and planned, and other information pertinent to NRC enforcement, in -
particular, we ask that you address your actions to ensure compliance with environmental
qualification requirements and the adequacy of your review of the implementation of associated
TMI related requirements. After the enforcement conference, you will be advised by separate
correspondence of the results of our deliberations.
