Dear Mr. Wallace:
lhis will acknowledge receipt of your letter dated October 14, 1993, in response to our Confirmatory Action Letter (CAL) dated September 16, 1993.
The subject of the CAL was the luss of off-site power event which occurred at LaSalle Unit 1 on September 14, 1993. Your letter provided the requested response for each of the items addressed in the CAL.
We have reviewed your response and have no further questions at this time.
However, as stated in the cover letter of the Augmented Inspection Team (AIT) report, the overall effectiveness of maintenance at LaSalle remains a concern.
Inadequate maintenance resulted in several equipment failures during the loss of off-site power event and recovery, and your currective actions for previous equipment failures were ineffective in preventing recurrence. We are aware that you are taking actions to evaluate and improve maintenance performance at LaSalle. We will closely monitor those actions and the actions described in your response to the CAL to determine if they are effective in correcting the root causes of equipment failures at LaSalle.
