This letter is written in response to Reference (b), which documents that our activities were not

i

conducted in full compliance with NRC compliance. The Violation, classified as Severity Level IV, was

l

identified during a routine safety inspection conducted from September 20-24,1993. Our response

!

'

to these items is provided below.

VIOLATION

(A)

10 CFR 20.1601(b) states in part that "in place of the controls required by paragraph (a)

of this section for a high radiation area, the licensee may substitute continuous direct

'

or electronic surveillance that is capable of preventing unauthorized entry."

I

(B)

Technical Specifications (TS) 6.5.B.1 states in part that "in lieu of the ' control device' or

' alarm signal' required by paragraph 20.1601(a)(1) of 10CFR20, each high radiation area

in which the intensity of radiation is greater than 100 mrem /hr at 30 cm, but less than

1000 mrem /hr at 30 cm, shall be barricaded and conspicuously posted as a high radiation

)

area and entrance thereto shall be controlled by requiring issuance of a Radiation Work

Permit (RWP)."

Contrary to (A) and (B) above, on September 21 and 22,1993, the inspector found two examples

of high radiation areas (one also a contamination area) which did not properly control access

to prevent inadvertent or unauthorized entry. The high radiation areas without proper access _

controls were found at the personnel entrance to the drywell and on the lower elevation of the

torus room. The duration of this condition was indeterminable.

The examples listed above constitute a Severity Level IV Violation.

RESPONSE

The two High Radiation Area access issues discussed in the inspection rcport were separate incidents

arising from two different r,auses.

!

i

in the case of the drywell personnel entrance, we incorrectly believed that the control point met the

i

requirements of 10CFR20. This was an interpretation problem which was corrected at the time of the

inspection.

In the case of the barrier that left a space between stanchions at the Torus area sump that could allow

,

i

unfettered entry, the cause was determined to be a lack of appropriately aggressive tours and

surveillances to find and rectify High Radiation Area barrier problems that may arise during outage

I

periods, coupled with a training weakness on such barriers. This was also corrected at the time of the

'

inspection.

The RP Manager will brief other department managers regarding the responsibilities of workers to not

inadvertently reposition RP barriers. Meetings have been held with our Radiation Protection (RP)

[

personnel to reinforce the importance of aggressive tours and timely corrective action in relation to

these issues. Additionally, formal training on the need for such oversight will be presented as part of

the RP Continuing Training program along with specific training on posting and barricade requirements

j

for High Radiation Aems. This training will be completed by December 31, 1993. In addition,

enhancements to our RP Self Assessment and Observation programs will provide improved oversight,

more detailed tracking of observations and timely feedback to all supervisors in such areas as High

Radiation postings and controls. As an additional step to improve our performance, the Radiation

'

Protection Manager will travel to another utility to benchmark ideas for further improvements to our

programs.
