REFERENCE:

This letter refers to your application dated February 5, 1990, for a Byproduct Material License. The following additional information is needed in support of your request 1.-
Under SAP-001 of your Administrative Procedures Manual, #3~.4, you refer to Consultants /Outside Agencies performing certain functions for your company.
You need to specify the minimum training and experience these individuals will have.
- Also, two of the functions -(Facility / Program Audits or Evaluations, and Field audits of radiographic operations / personnel) should be performed by individuals that have been spscifically named and their training and experience submitted to our office. As indicated in our licensing guide, these individuals should have at least one year experience in radiography using sealed sources.
This also needs to be corrected in SAP-008,#3.1.
Your procedures in SAP-008 a.hmild also. provide for radiographers that have not performed radiography in 3 months.
They should be audited during their first radiography operation.

Please clarify the training and experience of William E.
Parrish,.Jr.
It is not clear from the submission that your training included source retrieval and recovery, and your experience included at least one year. in. radiography with sealed sources.
3.
.Give the name and training and experience of the Radiation Safety Supervisor, since he will be sharing in the duties of the day-to-day radiation safety program.
.4.
Provide a description of the field (practical) exam that will be given to prospective radiographers.
5.
Identify.the individuals 'who will instruct and train prospective radiographers and radiographers' assistants.
Submit specific information about their qualifications including where, when and by whom they were trained in the l.

On the sample test and answer key submitted, there were a few errors.
On the Trainee's Examinatiuon, question 3 and 14 are the same.
On the Assistant Radiographer's Examination the answer to question 23 is wrong.
On the Radiographer's Examination the answer to question 8 was wrong, questions 3 and
- 5. are the same(Section 2), the answers to 6 and 7 in ll' section 4 are incorrect, the answer to 6 in Section 8 should specify that the source should be cranked back to the shielded position, and the answers to questions 1 and 2 in Section 10 i
contradict each other.
Also, I would recommend that the l
questions on the radiographer's exam not be identical to the assistant radiographer's exam, especially the math calculation problems.
Submit your corrections.

Under SAP-005,
- 4.2.2.A, it should be 5 rems instead of 4 rens.
Please correct.
I 8.
SAP-009 is not clear in who will be analyzing the leak test samples.
If you are using a kit and sending the samples off for analysis, indicate the kit model number and the name, address, and license number of the kit supplier.
If you are analyzing the samples yourself, specify the instrumentation that will be used for the measurement and its efficiency, and l
the method used to determine the activity on the sample.

SAP-014 does not appear to include the following records which need to be rethined for a minimum of three years.

Please include this clarification in your procedures.

Under Section 8, 3.6, your procedures should make it clear that if a person entering the retricted area does not respond to a shout or verbal warning, the source should be retracted immediately before physically trying to remove the person from the area.
This change also affects one of your questions on the exam..Please submit your changes.
