P Per our telephone conversation of this date I have hereby requested that the Quality Assurance Program for Atomic Inspection Labs, Inc. - Approval # 660 (see attached) be reinstated as of the date of your review. No changes have been made to the Q.A.
program since it's original approval with the exception of personnel changes at the Radiographer level of our organization chan.
I have enclosed with this request a check for $370.00 to cover the reinstatement application and ask that our certificate be faxed prior to being mailed. Our fax number is (505) 828 9846.
I appreciate your courtesy and quick response to this matter to allow my company to respond to a project next week under our reciprocity request which has been approved.
