REISSUE OF INSPECTION REPORTS NO. 50-254/93024(DRS) AND
NO. 50-265/93024(DRS) AND ASSOCIATED NOTICE OF VIOLATION
I am reissuing my November 12, 1993 transmittal letter, the attached
inspection reports (50-254/93024 and 50-265/93024), and the Notice of
Violation (NOV).
This action is necessary to more appropriately characterize
our concerns with maintenance activities and negative operator training at the
Quad Cities facility.
The original letter, while listing our concerns, did
not provide the operational framework and perspective to properly assess the
issues' significance.
The NOV is being reissued to correct the appearance that only one violation
was being issued when, in fact, three Severity Level IV's were being issued.
Please replace the November 12, 1993, letter and attachments with the enclosed
letter and attachments.
