Dear Mr. Stone:

l This letter e.Jdresses the inspection at your facility in Gaithersburg, Maryland, led by Mr. J. J. Petrosino of my staff on April 23-27, 1990, and the discussions of his findings with Mr. B. L. Meyers and other members of your staff at the conclusion of the inspection.

This inspection was performed to review activities regarding the control and quality of engineering services provided to NRC licensees by Bechtel Corporation.(Bechtel).

The primary area of concern involves multiple inaccuracies in piping and pi support stress calculations and isometric drawings (calculation packages)pethat have been identified in Bechtel calcula-tions performed at several nuclear p(ower plants in response to NRC Office of Inspection and Enforcerent Bulletin IEB) 79-14.

Areas examined during this-NRC inspection and our findings are discussed in the enclosed report.

This inspect 1on consisted of an examination of procedures calculational packages, and records of activities; interviews with personnel; and observations by the inspectors.

During this inspection, we found that the implementation of your quality assur-ance-(QA) program failed to meet certain NRC requirements.

Bechtel failed to.

effectively implement design control and design verification programs related to IEB 79-14 activities as described.in the enclosed Notice of Nonconfomance-(Notice).

Further, after the identification by Bechtel of deficiencies in stress calculation packages at~ Davis-Besse, Duane Arnold, Point Beach, and Palisades, it a1 pears that Bechtel did not address the deficiencies in order to determine if tiere was an underlying root cause(s) for the deficiencies at the multiple sites.

The deficiencies at Point Beach, Duane Arnold, and Palisades are technically similar to each other, and appear to be the result of an inadequately implemented QA program in engineering-services provided to NRC licensees.

-2 In the case of the Duane Arnold Energy Center (DAEC) deficiencies, no evaluation existed to indicate whether Bechtel considered the deficiencies for their cumulative significance on the DAEC.

Bechtel has provided similar 1EB 79-14 engineering support to a number of licensees including Arkansas Nuclear One (ANO), Callaway, Calvert Cliffs, Davis-Besse, Duane Arnold, Farley, Grand Gulf, Hatch, Hope Creek, Limerick, Millstone-3, Oconee, Palo Verde, Point Beach, Palisades, San Onofre, South Texas Project, Susquehanna Trojan, Turkey Point, and Wolf Creek.

During the inspection, the inspectors iacome aware that Bechtel compiled and sussnarized types of deficiencies in a paper which indicated that the Calvert Cliffs Nuclear Power Plant (CCNPP) piping and pipe support stress calculations could contain inaccuracies or errors.

This paper was then internally distributed to the majority of Bechtel's nuclear plant project managers under a l

March 27, 1990, interoffice memorandum from the Bechtel Corporation's Manager of Engineering. Although the NRC staff was generally aware of these issues, it is not clear from the paper the extent to which Bechtel was aware of specific deficiencies or the possibility of deficiencies at CCNPP.

In addition to the response requested below, Bechtel is requested to clarify Bechtel's understand.

ing and characterization of design deficiencies discussed in the March 27, 1990, interoffice memorandum referenced above.

The' specific inspection findings and references to the pertinent requirements are identified in the enclosures to this letter.

Please provide us within thirty days from the date of this letter a written statement containing:

(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventative measures were or will be completed. We will consider extending the response time if you l

can show good cause for us to do so.

The' responses requested by this letter and the enclosed Notice are not subject to the clearance procedures of the Office of Management and Budget as required i

by the Paperwork Reduction Act of 1980, Public Law No.96-511.
