Interim Report of 10CFR50.55(e) Item on Icose Holding Stem Nut Keys on Remote Operated Powell Valves (#72)

Dear Mr. Keppler:

On June 21, 1982, Detroit Edison's Mr. H.A. Walker, Supervisor - Construction Quality Assurance, telephoned Mr. P. Pelke of NRC Region III to report on a problem with loose holding stem nut keys on remote operated Powell valves at Fermi 2 site.

'IWo failures of Powell motor operated valves were experienced at the Fermi 2 site. The failures were caused by the stem nut holding keys falling out of place causing the valve not to operate. These Powell valves are model 19051-Y-WE, with Limitorque Operator SMB-5T.. The two valves are Fermi 2 valve -

number V8-2159 and V8-2160, and are in the E11-000 system.

Edison Engineering has discussed the matter with Powell Engineering, and they have agreed upon a temporary plan to correct the problem. Neither the cause nor a permanent. solution has been determined yet.

Another report on this item, either interim or final, is scheduled to be sent to you on or before November 17, 1982. If you have questions concerning this matter, please contact Mr. D. Ferencz, Supervisor - Construction Quality Assurance.
