SUBJECT: COMBINED INSPECTION REPORT NOS. 50-352/93-27 AND 50-353/93-27

During the period of November 1-5, 1993, the NRC conducted an announced inspection of

the Limerick Nuclear Generation Station licensed operator requalification (LOR) training

program. This inspection was conducted utilizing NRC Temporary Instruction (TI)

2515/117, " Licensed Operator Requalification Program Evaluation." This TI was developed

because of proposed rulemaking that would delete the requirement that the NRC examine

each licensed operator for purposes of license renewal. The purpose of this TI was to

evaluate the acceptability of the Limerick LOR training program through a performance-

based inspection in lieu of an NRC staff requalification examination oflicensed operators.

The LOR training program is important to safety to assure continued licensed operator

competency.

The inspection included a review of attributes similar to those normally evaluated during a

requalification examination, including the administration of the operating and written

'

examination. Additionally, selected programmatic aspects of the requalification training

program were inspected, which included, but were not limited to, a review of administrative

procedures and interviews with licensed operators and training personnel.

We conclude that the Limerick LOR training program maintains competent licensed

operators, since all operators examined passed a discriminating examination.

However, we are concerned that Limerick staff evaluation approach may be providing a

message to the operators that it is acceptable to depart from the procedures if the task is

accomplished. The requirement to observe facility operating procedures is specifically stated

in an individual operator license. A number of operators made significant mistakes using

facility procedures during the Job Performance Measures (JPM) portion of the operating test

In addition, the Limerick staff evaluation process appeared to accept these significant

operator mistakes. We understand your Limerick staff evaluation practice is that if the task

is accomplished and a minimum level of safety is not violated, the JPM will be graded as a

pass even if significant mistakes are made. Operations and training management expectations

are not considered in making JPM pass / fail decisions but are only provided as feedback to

the individual operator.

Given these observations, it would be useful for your staff to meet us within the next 60 days

to discuss the summary results of the Limerick requalification examinations, your staff's

perspective on operator mistakes using procedures, the root causes for these operator

mistakes, your management expectations in the evaluation process and any actions taken or

planned based on the assessment of the Limerick requalification examination results.

We received and hereby acknowledge your response to NRC Examination Report No.

50-352,353/93-18, dated November 1,1993. We thank you for your careful and thorough

review of the matter. Your response provides detailed actions that satisfy our concerns in

the area of procedures and training related to the power instability issue. Your cooperation
