Dear Mr. MacDonald:
Edwards Pipeline Testing, Inc. would like to terminate the Quality Assurance Program #0336 of Cleveland X-Ray Inspection, Inc.
Our parent company, International Testing Services, Inc., 363 N.
Sam Houston Parkway East, Suite 1270,
- Houston, Texas 77060 purchased the stocks of Cleveland X-Ray Inspection, Inc.,
in January of 1992.
We terminated Cleveland's NRC license #35-15205-01 on August 27, 1992.
I have an amendment dated September 17, 1992 that shows that Cleveland's NRC license was actually canceled.
On August 9th, 1993, we received Invoice #AMO6738-93 stating that Cleveland X-Ray Inspection, Inc. owed $1120 for their Quality Assurance Program.
Larry L. Lake wrote a letter to the License Fee and Debt Collection Branch on August 12th stating that we had terminated the Quality Assurance Program for Cleveland X-Ray on August 27, 1992 when we terminated the NRC license for Cleveland.
We believed this to have been the case.
Today, we received the final notice to pay this invoice.
We have found no amendment to prove that this Program has indeed been terminated.
(Larry joined the management staff of Edwards Pipeline in February of this year and I joined in April.)
Should we have an amendment in our file to show that Cleveland's Program has been terminated?
Do we owe this year's invoice for Cleveland X-Ray?
I am hoping you can help us in this matter.
