Storz Vice President Nuclear Davis-Besse c/o Toledo Edison Company 300 Madison Avenue Toledo, OH 43652 Dear Mr. Storz: The enclosed report refers to the inspection conducted by Mr. D. Shepard and Mr. C. Osterholtz of this office on November 1 - 5, 1993. The inspection evaluated activities at the Davis-Besse Nuclear Power Station authorized by NRC Operating License No. NPF-3. We discussed our inspection findings with Mr. S. Jain and members of your staff at the exit interview on November 5, 1993. The purpose of the inspection was to assess the licensed operator requalification program. The inspectors reviewed training administrative procedures, representative records, requalification training materials, and observed licensed operators during requalification examination administration. Additionally, the inspectors interviewed members of the operations and training staffs. Overall, the inspectors concluded that the licensed operator requalification training program was being implemented in accordance with 10 CFR Part 55 requirements. Based on the results of this inspection, certain of your activities appeared to be in violation of NRC requirements, as specified in the enclosed Notice of Violation (Notice). The violation, regarding the failure of a licensed operator to obtain a required medical evaluation, is of concern because of prior notification of problems with licensed operator medical examinations was provided in NRC Information Notice 91-08, issued February 5,1991, and you had ample opportunity to identify discrepancies in your own medical evaluation program. You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response. In your response, you should document the specific actions taken and any additional actions you plan to prevent recurrence. After reviewing your response to this Notice, including your proposed corrective actions and the results of future inspections, the NRC will determine whether further NRC enforcement action is necessary to ensure compliance with NRC regulatory requirements.
