SUBJECT:
RADIOLOGICAL CONFIRMATORY MEASUREMENTS INSPECTION AT QUAD CITIES

This refers to the routine safety inspection conducted by Mr. S. K' Orth of
i
.
this office on October 25 - 29, 1993. The inspection included a review of
authorized activities for your Quad Cities Nuclear Power Station, Units 1 and
'
2.
At the conclusion of the inspection, the findings were discussed with
those members of your staff identified in the enclosed report.
'I
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation of
-:
activities in progress.
In addition, our Mobile Laboratory was at the site
!
during the inspection to perform independent measurements of radioactivity.
I
Based on the results of this inspection, certain of your activities,
concerning the implementation _ of the radiological environmental monitoring
3
program (REMP), appeared to be in_ violation of NRC requirements, as specified _
'
in the enclosed Notice of Violation (Notice). 'Although the NRC has the
'
. ability to exercise discretion in licensee identified violations in Section-
'
VII.B. of the " General Statement of Policy for NRC Enforcement Actions,"
i
(Enforcement Policy, 10 CFR Part 2, Appendix C), this violation has been cited'
>
because of the extended duration of the violation and because your corrective
,
actions did not appear to address the root cause of the violation.
Certain
!
improvements have been noted in REMP oversight, but the extended duration for
which these activities remained unidentified by your staff indicated a lack of
station oversight in the program's administration.
Certain other of your activities, concerning the reporting of sample
collection deviations in the REMP, also appeared to be in violation ~of NRC
requirements, as specified in the enclosed Notice. This violation is of-
concern because it indicated a lack of openness with the Commission. The
information you reported to the Commission.in the 1991 and 1992 Annual

Radiological Environmental Operating Reports failed to properly describe the
deviations which had' occurred in your program and your corrective actions.
