SUBJECT: NRC INSPECTION REPORT 50-313/93-30; 50-368/93-30 (NOTICE OF
VIOLATION)
This refers to the inspection conductM by Mr. A. D. Gaines of this office on

November 1-5, 1993. The inspection included a review of activities authorized
for Arkansas Nuclear One, Units 1 and 2.
At the conclusion of the inspection,
the findings were discussed with those members of your staff identified in the
i
enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain t ? your activities appeared
to iie in violation of NRC requirements, as specified in the enclosed Notice of
Violation (M 'ce). The two violations are of concern because they involve
the failurr
Nilow radiation work permit requirements and the failure to
perform rad
-t + iurveys. Although the violation that involved the failure
to perform rad mon survays was self identified, the violation was a repeat
violation and, therefore, enforcement discretion allowed in the Enforcement
Policy was not enacted.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosures will be placed in the NRC Public Document Room.
The responses directed by this setter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
