SUBJECT: NRC Inspection No. 50-322/93-03
This letter transmits the results of safety inspections conducted by Mr. R. L. Nimitz and others
during the period August 1 - October 8,1993, at the Shoreham Nuclear Power Station, Wading
River, New York. The inspectors focused their attention on issues important to health and
safety, and based their findings on independent observations of on-going activities, interviews,
and document reviews. The inspection findings were discussed with you and members of your
,
staff periodically during the inspection and were summarized at the exit meeting on October 8,
1993, and with you by Mr. Nimitz on November 5,1993.
Areas reviewed during the inspection are discussed in the enclosed inspection report. The areas
reviewed included decommissioning status and activities; action on previous inspection findings;
organization, staffing, training and qualifications; radiological controls; radioactive waste
activities; maintenance and surveillance; quality assurance; fire protection and security. In
addition to these areas, detailed reviews of your planning and preparation for shipment of
slightly irradiated fuel to the Limerick Nuclear Power Station were conducted. The initial fuel
shipments were independently inspected by NRC personnel and independent radiation
measurements were made, as appropriate, to verify conditions prior to fuel transport. The
'
transfer of the shipping cask from the barge to a rail car at the inter-modal transfer point
(Eddystone, Pennsylvania) was also independently reviewed by the NRC. I.astly, the inspectors
reviewed your final close-out actions associated with the April 29,1993, fall of the refuelingjib
crane (RJC) (Reference NRC Inspection Report No. 50-322/93-02).
This letter also transmits the summary of the Management Meeting held at the NRC Region I
Office, King of Prussia, Pennsylvania, on September 8,1993. The meeting ~ was held to discuss
the transfer of fuel from the Shoreham Nuclear Power Station (Shoreham) to the Limerick
Nuclear Generating Station (Limerick). The meeting was attended by Mr. C. Giacomazzo, and

other representatives of Long Island Power Authority, representatives of Philadelphia Electric

and members of the public were in attendance. NRC management stated that, based on the
meeting and previous inspections, an r.ppropriate level of planning and preparation was provided
to enable the safe transfer of the fuel from Shoreham to Limerick.
The inspector's review indicated that, overall, decommissioning and termination survey activities
were conducted in accordance with the approved Decommissioning and Termination Survey
Plans. Training of personnel performing termination survey activities continued to be effective.
The inspector's review of quality assurance oversight during decommissioning indicated that,
overall, the oversight was of generally good quality. The use of QA auditors to evaluate
activities at the inter-modal transfer point was considered a very good initiative. However,
weaknesses in oversight of fuel shipping activities were identified by the NRC during review of
the initial fuel shipment at the Shoreham Station. The weaknesses were evidenced by NRC
identification of loose bolts on a neutron shield expansion tank hold-down bracket and
contaminated tools not indicated on shipping papers. Your staff took immediate actions to
address these matters, including appropriate revision ofprocedures to preclude recurrence. NRC
review of subsequent shipments did not identify similar concerns, indicating corrective actions
have been effective. Similarly, the inspector's review of the adequacy and implementation of
corrective actions associated with the fall of the RJC indicated those actions were effective.
Two unresolved items were identified and are discussed in the enclosed inspection report. The
first item involved conformance with the quality assurance requirements of 10 CFR Part 71.
The second item involved qualifications of personnel.
No violations were identified and no response to this letter is required. Your cooperation with
us is appreciated.
