Inspection Report 82-09
Dear Mr. Collins:
This letter is in response to the Notice of Violation received as a result of an inspection conducted at Fort St. Vrain during the period of April 30, 1982.
The specific items found in violation and the responses are as follows:
Technical Specification 7.4.a, " Procedures, Administrative Controls,"
states in part that, "... written procedures shall be established, implemented, and maintained...."
A.
Overall Plant Operating Procedure OPOP III, Issue 51, dated November 24, 1981, Section D,
" Preparation for Increasing Reactor Power to 2fs,"
states that, "6.
Verify SR-0P-12-M, Sealed Valve Check List, completed."
Surveillance SR-0P-12-M, dated June 18, 1981, requires V-6361 to be sealed open.
Contrary to the above, on April 15, 1982, at 9:55 a.m. MST, with reactor power at 32?4, the NRC inspector determined that V-6361 was not sealed and appeared to be closed.
The licensee later verified V-6361 to be closed (1) The corrective steps which will be taken and the results achieved:
V-6361 was sealed properly on April 15, 1982.

.. (2) The corrective steps which will be taken to avoid further violations:
SR-0P-12-M will be run every two (2) weeks rather than monthly for a period of six (6) months.
The increased surveillance frequency will provide better assurance that these incidents can be avoided.
(3) The date when full compliance will be achieved:

Administrative Procedure P-2,
" Equipment Clearances and Operation Deviations," Issue 4,
dated June 12, 1981, Section 4.1, under procedure for equipment clearances, states in part:
" REACTOR OPERATOR "3.
Fills out the required information on the Clearance Card and enters the card number on the Standard Clearance Points form and on all auxiliary tags.
Any Technical Specification encumbrances are noted in the Daily Log."
Contrary to the above, on April 29, 1982, at 1:15 p.m. MOT, the NRC inspector observed that two auxiliary tags for Clearance Number 3155, dated April 28, 1982, and placed on valves V-11119 and V-11319, were not numbered as required.
i This is a Severity Level V Violation.

l The auxiliary tags were immediately numbered properly showing Clearance Number 3155.
(2) The corrective steps which will be taken to avoid further violations:
j This matter was discussed with the individuals involved and the importance of following the procedure stressed.
Our Quality l
Assurance Department's monthly surveillance of the outstanding l
clearances has not revealed a trend of similar problems and we l
believe this to be a rather isolatad incident. As fu-ther verification, an expanded informal audit of the clearance by the Shift Supervisors has been implemented.
