Dear Mr. Link:

This refers to the routine safety inspection conducted by Messrs. K. R. Jury and J. Cadzala of this office, from September 7 through October 25, 1993, of activities at the Point Beach Nuclear Plant Units 1 and 2.

Our findings were discussed with Mr. G. J. Maxfield and members of his staff at the conclusion of the inspection.

Areas examined during the inspection are identified in the report.

Within' these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations.

During this inspection, certain of your activities appeared-to be in violation f

of NRC requirements, as specified in the enclosed Notice. One violation concerns a service water system isolation valve being inoperable for a time in excess of that allowed by technical specifications.

This violation was in part related to a lack of a questioning attitude by maintenance personnel.

Although this violation meets the criteria for mitigation of enforcement j

sanctions, it is nonetheless being cited due to the length of time the valve was inoperable and the numerous missed opportunities _ to correct the fault.

A second violation with three examples concerns inadequate instructions for performing maintenance activities. These violations are of concern because g

they indicate a breakdown in the maintenance work and review process and t

continuing weaknesses in communications.

j An additional violation concerns a failure to perform post-maintenance testing. However, as described in the enclosed inspection report, you identified this violation. Therefore, the violation will not be subject to enforcement action because your efforts in identifying and correcting it meet the criteria specified in Section VII.8 of the " General Statement of Policy-and Procedure for NRC Enforcement Actions," (Enforcement Policy 10 CFR Part 2 Appendix C).
