l t Office of Nuclear Reactor Regulation Items of Interest Week Ending November 19, 1993 Susouehanna Steam Electric Station. Units 1 and 2 On October 28, 1993, the licensee stopped reloading Unit 1 to investigate several refueling events. At the time, only 52 of the 764 fuel assemblies had been loaded in the reactor. In accordance with the Confirmatory Action Letter i issued on October 29, 1993, refueling activities were put on hold. A management meeting was held in Region I on November 18, 1993, during which Pennsylvania Power and Light presented their assessment of the events and corrective actions. The augmented inspection team completed all formal onsite activities as of November 5, 1993. A public exit meeting will be held onsite on November 22, 1993. Beaver Valley Unit I was at 7% power preparing to restart on November 17, 1993, after having tripped on a loss of offsite power on October 12, 1993. The licensee took the opportunity presented by the outage to inspect and make repairs in several systems. Unit 2 remains shutdown under a Confirmatory Action Letter. An augmented inspection team (AIT) left the site on November 13, 1993, after investigating a common mode failure of EDG load sequencers. The failure occurred during a surveillance test just prior to restart from a refueling outage. The licensee added voltage surge suppressors at certain relays to correct the problem. The first integrated system test with the surge suppressors in place failed, apparently because inadequate test instruments were used. The AIT plans to review the test prior to NRC approval of restart. The licensee expects to submit a formal request later this week for restart approval. Severe Accident Mitiaation Desion Alternatives (SAMDAS) The Probabilistic Safety Assessment Branch and other staff from NRR and RES met with TVA on November 10, 1993, to discuss the preliminary results of an updated Individual Plant Examination (IPE) analysis for Watts Bar, and TVA's analysis of Severe Accident Mitigation Design Alternatives (SAMDAs). The i estimated core damage frequency (CDF) for Watts Bar, previously among the highest in the country, has been reduced by a factor of 4 (from 3E-4 to 9E-5). The reduction was achieved via configurational changes, analytical refinements, and procedural modifications. Examples of these changes include changing the configuration and the use of more realistic success criterion for the component cooling water system, a key contributor to Watts Bar risk. A number of issues were raised in the meeting, among which is the IPE modeling of pump seal LOCAs. TVA will respond to these issues in a meeting in December. Since the staff will use the Watts Bar IPE information in conducting SAMDAs evaluations, the staff identified a number of issues that need clarification j i l c 5 NOVEMBER 19, 1993 ENCLOSURE A

1.~ + P 7 by the TVA IPE team. These issues were discussed in detail in the meeting and TVA response is expected in the forthcoming December meeting. TVA is also planning to submit an updated Watts Bar IPE in December 1993. Workshoo on Environmental Qualification On November 15-16, 1993, the NRC held a Workshop on E. /ironmental Qualification of Electric Equipment at the Holiday Inn Crowne _ Plaza in '1 Rockville, Maryland. The workshop sponsored by the Office of Research had approximately 250 participants. The attendees included representatives of licensees, vendors, test laboratories, national laboratories, the United i Kingdom, the Union of Concerned Scientists, the Nuclear Utility Group on Equipment Qualification (NUGEQ), NUMARC, NIST, EPRI, and NRC. The meeting focused on the technical issues associated with environmental qualification (EQ) and included plenary sessions on the first day and the afternoon of the second day. Breakout sessions were held the morning of the second day to focus on the topics of Preaging/ Preconditioning; EQ Operating Experience; Condition Monitoring Techniques; and EQ Testing. Th'. workshop provided open discussions between the industry and NRC regarding current questions in'EQ and the activitias being performed under the NRC EQ Task Action Plan (TAP). RES will use information gained during the workshop to develop a program plan for research work to be performed in support of the EQ TAP. t 4 5 1 l c l t NOVEMBER 19, 1993 ENCLOSURE A 4

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending November 19, 1993 Westinghouse Assessment Meeting Regional and Headquarters staff met with Westinghouse Commercial Nuclear fuel Division's management on November 5, 1993, to review the licensee's performance under its materials license SNM-1107. The meeting was held at the licensee's Columbia, South Carolina plant. Westinghouse presentations included descriptions of their Safety Margin Improvement Program for nuclear criticality safety and other areas of licensee performance. The Nuclear Regulatory Commission staff and Westinghouse were in general agreement on the areas of Westinghouse performance that represented strengths, as well as on those that called for improvement. Covernment of Japan Visit ]n response to a request by the Government of Japan, in conjunction with the US-Japanese informal bilateral discussions of IAEA Safeguards, a Japanese delegation toured the Westinghouse LEU fuel fabrication plant in Columbia, South Carclina. This plant has been the site of IAEA experimental application of the technique "Short Notice Random Inspection" verification of shipments and receipts. Part 70 St andard Review Plan Meetinas The Nuclear Regulatory Commission is developing a Standard Review Plan to provide guidance to staff members who review applications for licenses, under 10 CFR Part 70, to produce enriched uranium or to process it in making reactor fuel. During November 1993, the staff has held a series of public meetings with f uel cycle licensees to obtain information relevant to the development of acceptance criteria for the various areas of review within the Standard Review Plan. On Monday, November 15, the topics of management control and oversight and quality assurance were addressed. The next day the series of meetings concluded with a discussion of nuclear criticality safety. Representatives of contractor organizations and about a dozen industry representatives attended and provided useful comments concerning the acceptance criteria. The Standard Review Plan is scheduled to be available in August 1994, to accompany the proposed revision of 10 CFR Part 70. Periodic Quality Assurance Meetinq On November 16, 1993, staff from the Nuclear Regulatory Commission Division of High-level Waste Management held another in the series of periodic quality assurance (QA) meetings with the staff of the U.S. Department of Energy (DOE), Office of Civilian Radioactive Waste Management (OCRWM), to discuss issues of mutual interest related to the QA program for the DOE high-level waste repository. The meeting was attended by representatives of the State of Nevada; Clark County, Nevada; Nye County, Nevada; U.S. Geological Survey, Nuclear Waste Management System Management and Operating Contractor; Science Applications International Corporation; QA Technical Support Services; L ; ' l NOVEMSER 19, 1993 ENCLOSURE B

The site characterization plan (SCP) Issues Hierarchy and Controls for Resolution of SCP Issues were i also discussed. The Nye County representatives discussed QA plans for a drilling program independent of DOE's program. 1-Meetino of the Nevada Leaislature's Committee on Hiah-level Radioactive Waste t On November 12, 1993, the On-Site Representatives (ors) met with the Nevada l Legislature's Committee on High-Level Radioactive Waste in Las Vegas, Nevada. The purpose of the meeting was to provide an update and status of the Nuclear Regulatory Commission's review of the Department of Energy (DOE) scientific and technical studies and to explain any areas of concern or dispute. The Committee Chairman also requested that representatives of the State and local i governments, and DOE discuss these same topics from their points of view. At { the Chairman's request, the ors described the role of the NRC, and how it is implementing its responsibilities in the High-Level Radioactive Waste program, i for the benefit of four freshman members on the Committee. j State Supreme Court Favorable Rulina on the North Carolina low-level Radioactive Waste Manaaement Authority and its Contractor. Chem-Nuclear l Systems. Incorporated On November 5,1993, the Supreme Court of North Carolina ruled in favor of the North Carolina low-Level Radioactive Waste Management Authority and its j Contractor, Chem-Nuclear Systems, Inc., in lawsuits brought by the North j Carolina counties of Richmond, Chatham, and Wake. These suits were begun in -l 1990, after the Authority designated sites situated in Richmond, Wake, and j Chatham Counties as potentially suitable sites for a planned low-level radioactive waste disposal facility. Both the Superior Court and the Appellate Court previously had dismissed claims brought by the counties. l These claims charged that the Authority and Chem-Nuclear had not complied with j j site selection requirements in state law;- that the site selection process ) itself was flawed, and that an Authority member had an undisclosed conflict-of j interest. North Carolina Supreme Court considerations were limited to a i single issue raised by a dissenting Appeals Court Judge involving an earlier case that addressed the siting of a hazardous waste facility. The Court held, in the earlier case, that the State's Waste Management Authority may not be preliminarily enjoined in its process of site selection until the Authority has completed the permitting process and made a final site selection. i l Office of Nuclear Regulatory Research Items of Interest 4 Week Ending November 19, 1993 l Comment Period Extension for Two Reactor Pressure Vessel Draft Reaulatorv Guides (DG-1023/DG-1025) l A Federal Register announcement was sent to publish the notice of extension on comment period for the draft Regulatory Guides DG-1023, " Evaluation of Reactor Pressure Vessels With Charpy Upper-Shelf Energy Less Than 50 Ft-Lbs", and DG-1025, " Calculational and Dosimetry Methods for Determining Pressure l Vessel Fluences." The comment period was extended from December 17, 1993, to l January 31, 1994, to accommodate a request submitted by NUMARC. The request l noted that the NRC staff is holding a meeting on December 9, 1993 to discuss ~ the two guides and that insufficient time exists from this meeting date to December 17, 1993, to develop comments and to adequately coordinate comments. l Workshop on Environmental Oualification of Electrical Eauipment On November 15-16, 1993, the Division of Engineering of the Office of Nuclear Regulatory Research sponsored a workshop to discuss the environmental qualification (EQ) of electrical equipment used in nuclear power plants. Approximately 250 persons attended the workshop in Rockville, Maryland, l which was organized by Brookhaven National Laboratory (BNL). The nuclear utility industry was well represented with 75 participants from 40 utilities, three from EPRI, and three from NUMARC. Seventy-seven experts in the area of EQ from the nuclear industry, including testing laboratories, equipment manufacturers, architect engineers, and consultants actively participated. Attendees from the NRC staff included RES, NRR, AEOD and the Regions. Other l 2 attendees were affiliated with the Department of Energy (DOE), the National Laboratories, the National Institute of Standards and Technology, the United l Kingdom, and Canada. The principal. purpose of this workshop was to solicit information that the staff could utilize to develop the EQ Research Program Plan. The discussions i d focused upon technical issues related to environmental qualification of electrical cables. This was successfully accomplished through the use of prepared presentations by experts in the field, and by four concurrent " breakout sessions" which provided an opportunity for all attendees to i contribute and discuss specific technical issues. The primary areas addressed in the plenary session of the workshop were the NRC's perspective on EQ informational needs, and questions dealing with the technical aspects of environmental qualification, such as p) accelerated ' aging techniques-to i simulate the natural aging of electrical equipment, (2) methods to monitor the condition of cables, (3) operating experience associated with EQ equipment, and (4) EQ testing methods. These four areas were the subject of detailed discussions during the four breakout sessions which occurred on the second day of the workshop. The workshop included an opportunity for an open exchange of ideas and i information between industry personnel, researchers, equipment manufacturers, and regulators involving EQ issues, descriptions of state-of-the-art activities in condition monitoring and research techniques, discussion of 1 l i NOVEMBER 19, 1993 ENCLOSURE C { )

_.-a I i 2 l i several recent equipment failures and their causes experienced at operating i facilities, and presentations describing current licensee actions related to j monitoring normal service conditions such as on line temperature monitoring in specific plant locations. The workshop participants noted that there are no condition monitoring techniques currently available which are capable of l determining the extent of cable degradation in the context of remaining qualified life. Several participants discussed a concern that any testing i could lead to additional regulatory burden on licensees. l Hualien Larae Scale Soil-Structure Interaction Experiment [ On November 1-4, 1993, Herman Graves, SSEB/DE, attended the meetings of the Executive Oversight and Technical Management Committees for the Hualien large l Scale Soil-Structure Interaction Experiment. This experiment is-a cooperative effort by Taiwanese, Japanese, French, Korean, and American (NRC & EPRI) participants to validate soil-structure interaction codes and models. The meetings were hosted by the Korea Institute Of Nuclear Safety and the l Korea Electric Power Corporation in Taejon, Korea. The purpose of the meetings was for participants to discuss their predictions of the Forced Vibration Test (FVT) of the 1/4 scale model reinforced concrete containment l and analytical methods used. Discussions centered on the results of the actual FVT and how they compared with predictions. Also discussed was an earthquake of Richter magnitude 4.1 and ground acceleration of less than 0.19 which occurred on September 15, 1993, near the Hualien, Taiwan, test site. j in addition to committee meetings, the participants visited the Wolsung Nuclear Power Plant. Wolsung has one operating unit, a second unit has a i nearly complete containment building, and the foundations for units 3 and 4 have been prepared. Thermal-Hydraulic Codes RELAP5/ MOD 3.1.1 was released during the week of October 25, 1993, to NRC staff and contractors. MOD 3.1.1 contains all of the improvements added to RELAP in the last 18 months to aid staff in evaluating the safety of the AP600 and l SBWR plants. Among the modifications made to the prior released version RELAP5/ MOD 3.1 are an improved separator model, level tracking, condensation l in the presence of noncondensables, code speedop to aid in calculation of long-term transients, boron transportation and thermal stratification. RELAP5/ MOD 3.1.1 has also been interactively coupled to the CONTAIN code to improve analyses of the containment effects on the passive cooling systems in the AP600 and SBWR designs. RELAP5/ MOD 3.1.1 will undergo developmental. i assessment prior to its release for use outside of NRC in the Fall of 1993 with the designation of RELAP5/ MOD 3.2. a NOVEMBER 19, 1993 ENCLOSURE C -I .. ~

~ Office for Analysis and Evaluation of Operational Data ~ Items of Interest + Week Ending November 19, 1993 Diaanostic Evaluation and Incident Investiaation Branch (DEllB) f On November 17, the Quad Cities Diagnostic Evaluation Team report was signed by the EDO and forwarded to the licensee for their review and response. i Incident Resoonse Branch (IRB) IRB staff participated in an Expert's Group meeting at the International + Atomic Energy Agency in Vienna, Austria to help in the preparation of a [ document on " Emergency Planning and Preparedness." 4 On November 17, IRB staff coordinated a meeting with representatives of American Nuclear Insurers and the States of Minnesota (representing the National Emergency Management Association), Ohio, Pennsylvania and Virginia to discuss financial resources for emergency response. Representatives from NRR i and OCG also participated in the meeting. On November 19, IRB staff participated in a meeting with NMSS and the State of Maryland to discuss the results of a recently completed radiological survey of the neutron products facility in Dickerson, Maryland IRB staff and NUS have begun upgrading the Emergency Response Data System (ERDS) software to rur on an improved version of the operating system. Testing will continue through next week. t Trends and Patterns Analysis Branch (TPAB) i i Fred Manning presented a paper entitled, "An Interpretation of Accident l Precursor Data," on November 16, at the Winter Meeting of the American Nuclear Society in San Francisco. The paper used a Complementary Cumulative Distribution function (also referred to as a frequency of Exceedance Curve) to interpret sparse precursor data. i i 4 Office of Information Resources Management l ~ Items of Interest Week Ending November 19, 1993 a I Conaressional Affairs Action Items Trackino System j On November 10, 1993, the Office of Information Resources Management (IRM) implemented an Action Item Tracking System for the Office of Congressional Affairs (OCA). The application was designed to be used as an aid to control the many requests that OCA receives from Congress through a tickler based mechanism. The system also tracks the assignments of staff and produces reports of monthly activities. Further, the system supports a " key topics" index reference feature whereby OCA can quickly get to the status of requests for specific items or from a specific Congressman. i Special Pro.iect - IRM Review of RES Files j During the months of October and November 1993, IRM and Office of Nuclear l Regulatory Research (RES) representatives met so that IRM could assist RES with their files to determine the documents that are eligible for destruction or transfer to the NRC's Archival Facility, Columbia, MD. This-is in preparation of the transfer of the RES programmatic records to IRM for maintenance with the 1994 move to Two White Flint North. Between October 26, i 1993 and November 16, 1993, IRM has assisted three RES divisions and.has i reviewed approximately 90 cubic feet of material. This effort will continue until all of the RES files have been evaluated. f FTS 2000 i A meeting was held between the Telecommunications Branch and FTS 2000 representatives to discuss alternate routing from Two White Flint North to the FTS 2000 network to provide enhanced reliability for the White Flint complex. Currently, all voice and data FTS 2000 traffic from One White Flint North 2 routes through the Montrose Central Office. Office of Personnel Items of Interest Week Ending November 19, 1993 Employee Assistance Professionals (EAP) Association National Conference From November 13-17, 1993, Employee Assistance Program staff attended the EAP Association National Conference, in Anaheim, California. The conference featured five tracks: Direct Services, Labor, Administration / Policy, Clinical / Resources, and Research. Workshop topics included Violence in the i Workplace, Treatment of Crack Cocaine Addiction, Accommodation of Employees with Disabilities, Critical Incident Stress Debriefing, and Health Care Reform in the Areas of Mental Health and Substance Abuse Treatment. Great American Smokeout of 1993 at NRC The Employee Assistance Proaram commemorated The Great American Smokeout of 1993 by videotaping the successful experiences of several NRC employees who quit smoking. The purpose of the video was to encourage others who may be struggling with their own nicotine addiction. The film was shown continuously during the lunch hour in the OWFN lobby. In addition, handout materials from the American Cancer Society were available, as well as a sign-up sheet for those interested in enrolling in the next NRC Smoking Cessation Program _ planned for January 1994. Office of Enforcement Items of Interest Week Ending November 19, 1993 { Sianificant Enforcement Actions l An Order Modifying and Suspending License (Effective Immediately) and Demand for Information was issued November 16, 1993, to Indiana Regional Cancer Center, Indiana, Pennsylvania. In addition to suspending the license, the license is modified to remove Dr. James Bauer, Radiation Safety Officer and sole authorized user, from engaging in activities under the license. This I action is based on recent inspection findings as well as prior incidents in which Dr. Bauer had been involved. The URC has serious concerns about compliance with NRC regulatory requiremer.ts at Indiana Regional Cancer Center i given: (1) Dr. Bauer's performance of activities with a strontium-90 source that were not authorized by the license, (2) Dr. Bauer's failure to provide complete and accurate information to NRC inspectors, and (3) Dr. Bauer's failure to cause an adequate survey to be made which resulted in a November. 22, 1992, event at the Indiana Regional Cancer Center. The licensee or any affected person has twenty days from the date of the Order to request a hearing. A separate Demand for Information is also being issued today to Dr. Bauer concerning his future participation in licensed activities. (EN 93-114) A Notice of Violation and Proposed Imposition of Civil Penalties in the amount of 56.250 was issued on November 17, 1993, to Hahnemann University, Philadelphia, Pennsylvania. This action was based on two Severity Level III violations. The first violation involved a substantial failure to implement the Quality Management Program, as required by 10 CFR 35.32, in that written policies and procedures were not adequately established, which led to written directives not being prepared, or prepared improperly, prior to administration of radioactive material. The second Severity Level III violation involved the failure of the RSO to ensure certain specific requirements were met, thereby representing a breakdown in the control of licensed activities at the facility. There was some cleaning and touch-up work on Floors 4 through 10 as OMNI prepared for a GSA/NRC walk-through of the space scheduled to begin on November i 22. On November 17, the Office of Consolidation transmitted final floor plan changes to GSA for Floors 7 and 8. On November 17. Frank Gambino of Lerner Enterprises introduced the design team for the TWEN Plaza level (Floor 1) to the Office of Consolidation and GSA project staffs. The Weihe Partnership will provide architectural and interior design services. Cini-Little International will provide kitchen and cafeteria design assistance and equipment specification. Lee D. Eisenhower (LDE) will provide mechanical and electrical design and specification. A construction contract based on competitive bidding will be let by Lerner Enterprises later. Chairman Selin and Executive Director for Operations James Taylor met with the Executive Board on November 17 to discuss common issues of interest. j NARUC Annual Convention t The National Association of Regulatory Utility Commissioners (NARUC) held its Annual Convention on November 14-18, 1993 in New York, New York. Commissioner Rogers, accompanied by Myron Karman, Legal Assistant, and Spiros Droggitis, State Programs, participated in the Subcommittee on Nuclear Issues / Waste Disposal meeting on November 14. Commissioner Rogers provided the Subcommittee with an update on NRC activities in the areas of spent fuel storage, decommissioning and license renewal. Daniel Dreyfus, Director of the.0ffice of Civilian Radioactive i Waste Management, provided the Subcommittee with a s +.a t u s report of the l Department of Energy's (DOE) waste program and participated in an evening workshop on interim storage of spent fuel. Dr. Dreyfus indicated that his i initial emphasis will be to take measures to increase the waste program budget i above the present $390 million level. Dr. Dreyfus also indicated that DOE l Secretary O' Leary was reviewing the 1998 fuel acceptance issue. The Subcommittee adopted a resolution (later adopted by NARUC) which calls for continued j development of multi-purpose casks. A representative from the Public Utility l Commission of Ohio made a presentation to the Subcommittee in which he proposed that States join together by pooling resources and expertise in reviewing the designs of the next generation of nuclear power plants in the NRC design certification process. The NARUC Assistant General Counsel reported to the Committee on Electricity that NARUC intended to request reconsideration of 'a recent Federal Energy Regulatory Commission order which retained. conservative investment rules for utility investments in nuclear decommissioning trusts. Commissioner Remick signed the agreement with Dr. Naschi. Shioment of Yankee Rowe Steam Generators i The first two of four steam generators (SGs) from the Yankee Rowe facility began tha rail portion of the transfer to the Barnwell, South Carolina, waste disposal site on November 17. The SGs were cleaned and j filled with an expanding grout to fix all remaining internal contamination prior to shipment. On November 16 and 17 they were i transferred one at a time about six miles by heavy hauler from the plant j site near Rowe, MA, to the railhead, and loaded onto rail flatcars. 1 The rail transfer to Barnwell for burial will be by exclusive _ use train and will require about 72-hours. Region I radiation protection and transportation specialists determinei that the shipment met regulatory l requirements during the transfer to the railhead and after being loaded to the railcars. Approximately 75 protesters peacefully accompanied the l transfer to the railhead of the first SG on November 16. There was' also- { considerable media coverage of that transfer. South Carolina Electric and Gas Company j l On November 16, representatives from the South Carolina Electric and Gas Company were in the Region II Office to attend a Management Meeting to discuss Engineering issues at their Summer facility. I 2. Southern Nuclear Oo_gratina Company On November 17, representatives from the Southern Nuclear Operating Company were in the Region II Office to attend an Enforcement Conference l related to configuration control problems at their Farley facility. This conference was open for public observation. i 3. Duke Power Company i ~ On November 17, representatives from the Duke Power Company were in the Region 11 Office to attend an Enforcement Conference regarding the Augmented Inspection Team findings at the McGuire plant. -{ 4. Nuclear Fuel Services (NFS) - Damaaed Shionino Cask Hinae l On November 16, during an inspection of a SuperTiger shipping cask before shipment, the NFS identified a crack cn one of_four. hinges on the' door to the package. The package door is held in place by 10 bolts-during' shipment; the hinge is to facilitate opening and closing the door during loading. It appears that the four hinges were not well aligned, l and when the door was opened beyond 90 degrees, this stressed the hinge, leading to its failure. This cask has had cracked hinges in the past, in 1980 and 1983. (This is the first time NFS has used this particular .l package.) NFS had inspected the hinges as part of the pre-loading inspection, and had not identified a problem with the hinge during that ' l inspection. NFS unloaded the cask, and will not use it for shipment at-this time. Region 11 in conjunction with NMSS issued a Confirmatory Action Letter on November 18, confirming that NFS will not use Superliger packages until receiving NRC agreement. ] '5. U. S. Navy Severity Level III Violation [ On November 18, the Region issued a Severity Level III Notice of Violation to the U. S. Navy for record falsification by the Shore l Intermediate Maintenance Activity in San Diego, California in 1990. Records were falsified to show that radiation dosimetry had been returned to the Navy Dosimetry Center for processing when it had not. 1 The Navy conducted an investigation into the. situation, and took l corrective action based on the investigation findings. Wallace, Chief Nuclear Operating Officer, Commonwealth i Edison Company. The purpose of the meeting was to review the current status of Commonwealth Edison Company site activities and improvement program. 2. Zion Station On November 18, 1993, the Zion Review Team, led by the Deputy Director, Division of Reactor Safety, completed a three-day site visit. The team examined operations, maintenance, engineering, planning and scheduling, and management actions with a fLtus on outage performance in these areas. The team concluded that while overall performance continues to improve, management attention needs to be increased for system engineering, development of more aggressive goals in maintenance backlog control, and use of the management action plan system. 3. Davis Besse On November 16, 1993, the Director, Division of Reactor Safety, conducted a service water system operational performance inspection exit j at Davis Besse. Although the team did not identify any design or i performance problems related to the service water system, numerous discrepancies in P&lDs, operations, schematics, and operational i procedures questioned the adequacy of the licensee's configuration control process. Since 1988, Davis Besse's Quality Assurance organization has identifie.: configuration control problems as a persistent recurring issue. 4. Duane Arnold Nuclear Plant On November 19, 1993, an enforcement conference was conducted in the Region 111 office between representatives of Iowa Electric Light and Power and members of the Region 111 staff to discuss a technical specification violation relating to inoperability of a standby diesel generator. 5. Dresden Nuclear Power Station i On November 17, 1993, individual enforcement conferences were held in i the Region 111 office with four persons involved in a September 18, 1992, Dresden control rod mispositioning event. Persons attending included the on-shift Senior Reactor Operator, Reactor Operator, and two Nuclear Engineer Trainees. Dispatch of Auamented Inspection Team at fort Calhoun Station An Augmented Inspection Team (AIT), consisting of members from Region IV and the Office of Nuclear Reactor Regulation, was dispatched to Fort Calhoun Station to investigate rod control system problems which have resulted in unexpected rod motion. The Fort Calhoun Station is nearing the conclusion of its refueling outage. A Confirmatory Action Letter 7 was issued to confirm the licensee's commitment not to restart from the outage until it has briefed the NRC staff on the results of efforts to identify and correct the problems.
