APPENDIX A NOTICE OF VIOLATION Florida Power & Light Docket Nos. 50-250 & 50-251 Turkey Point 3 and 4 License Nos. DPR-31 & DPR-41 As a result of the inspection conducted on February 26 - April 2,1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),

the following violation was identified.

Technical Specification 6.8.1 requires that written procedures and admini-strative policies be implemented and maintained.

Contrary to the above, 0.P.16125.1, " Drawing Down the Refueling Cavity Using the Residual Heat Removal System" was not maintained and A.P.190.15,

" Plant Projects - Approval, Implementation and Regulatory Requirements" was not fully implemented in that:

0.P. 16125.1 was not updated to reflect the required position of valves 870A and 870B prior to commencing the drawing down of the Unit 3 refueling cavity on thrch 17, 1982; and procedure A.P.190.15 was not fully implemented in that as of March 31, 1982, the following steps of the procedure were not performed, as required, for Plant Change /flodification (PC/ft)80-101, implemented on December 7,1981:

a.

Step 5.3.2:

requires that marked up drawings, special test, imple-mentation and acceptance procedure be submitted to the plant Technical Department within 20 working days after PC/11 Work Completion.

b.

Step 5.3.3:

requiring " written notification of PC/M Work completion" to the Technical Department PC/ft Coordinator within 5 working days af ter completion date.

c.

Step 8.2.4:

requiring that the entire PC/M package for each completed PC/f1 be forwarded to the Technical Department PC/f1 Coordinator "within 60 days of completion of the hardware changes authorizes by the PC/ft".

d.

Step 5.4:

requiring update of procedures affected by PC/Ms (Procedure ONOP 208.9, "Aunnunciators Panel G-fliscellaneous" and itP 14007.20,

" RUST Instrumentation Calibration" had not been updated and/or distributed at the time of the #4 RWST overflow on thrch 17,1982).

Step 8.3.3:

requiring the generation of a " Functional Test and Acceptance Procedure" that canplies with the criteria in step 8.3.1 of AP 190.15.

This is a Severity Level IV Violation (Supplement I.).
