SUBJECT: NRC INSPECTION REPORT 50-298/93-25 (NOTICE OF VIOLATION)

!

This refers to the inspection conducted by Mr. R. A. Kopriva and the inspectors

identified in the enclosed report, of this office, on August 29 through

October 9, 1993. The inspection included a review of activities authorized for

your Cooper Nuclear Station facility. At the conclusion of the inspection, the

findings were discussed with you and those members of your staff identified in

the enclosed report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of activities

in progress. The purpose of the inspection was to determine whether activities

authorized by the license were conducted safely and in accordance with NRC

requirements.

Based on the results of this inspection, certain licensed activities appeared to

be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The first violation is of concern because an alarm response

procedure specified that the high pressure coolant injection system be secured

for an alarm condition which could be encountered during a safety injection

,

actuation but would not jeopardize the systems' integrity or effect its

performance.

The second violation is of particular concern because the

l

procedural deficiency, which the NRC staff had promptly reviewed with management,

l

was not promptly corrected and subsequently contributed to another event where

'

the high pressure coolant injection system was secured when the alarm annunciated

l

during a surveillance test.

l

In addition the staff is very concerned with the as-found conditions inside the

i

safety-related starter racks and motor control centers.

The conditions found

l

indicate that plant personnel accepted substandard work practices during the

l

outage and that management oversight and supervisory involvement was not

l

adequate.

,

l

A management meeting, open to the public, has been scheduled for November 23,

'

1993, at the NRC Region IV office to discuss your ongoing industry information

i

program implementation improvements.

specified in the enclosed Notice when preparing your respcnse. In your response,

you should document the specific actions taken and any additional actions you

plan to prevent recurrence.

After reviewing your response to this Notice,

including your proposed corrective actions and the results of future inspections,

the NRC will determine whether further NRC enforcement action is necessary to

ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this

letter and its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject to

l

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
