MILLSTONE COMBINED INSPECTION NOS. 50-245/93-19;
50-336/93-14; AND 50-423/93-15
This refers to the safety inspection conducted by Mr. P. Swetland and others rithis office on
June 30,1993 through August 17,1993, at Millstone Station in Waterford, Connecticut.

The preliminary nndings were discussed with Mr. S. Scace, the Unit Directors, and others of
your staff at the conclusion of the inspection. Areas examined during the inspection are

described in the enclosed report. Within these areas, the inspection focused on issues
important to public health and safety, and consisted of performance observations of ongoing
activities, independent veri 6 cation of safety system status and design configuration,
interviews with personnel, and review of quality records.
The events surrounding the August 5,1993, Unit 2 plant shutdown and subsequent plant
start-up raised serious NRC concern as to the level of operator attention to and management
~
oversight of these activities. In addition to the work control and management oversight
de6ciencies regarding the leak repair activities which caused the plant shutdown, failures in
work control and plant con 6guration control during the outage resulted in a reactor cooldown
system draindown event and a main steam leak inside containment during the start-up.
Opemtor inattention to the feedwater system caused an automatic reactor trip during the start-
up. As a result, you commissioned several task groups to review the causes of these
problems and implemented several immediate corrective actions to assure the safety of
continued operations. The issues concerning the poor maintenance practices which caused
the shutdown have been documented in NRC Inspection Repon 50-336/93-18. The other
events are documented herein and NRC follow-up of your ongoing corrective actions will be
the subject of future NRC inspections.

During this inspection, we also evaluated your self-identification and correction of problems
l
related to missing penetration fire barriers at Unit 1, late inservice tests at Unit 2, service
water system work control process problems at Unit 3, and inadequate foreign material
!'
exclusion at the Unit 3 spent fuel pool. We determined that your corrective actions for these
.
issues were acceptable and the problems individually posed minor safety consequences.
Therefore, m accordance with Section VII.B of the NRC Enforcement Policy, enforcement

discretion was exercised and violations were not cited for these cases. Regarding the Unit 3

service water obstruction, due to the safety importance of this system, your attention is
warranted to assure that it remains free from design and operational vulnerabilities in the
i
future.

During this inspection, we evaluated your corrective actions regarding the three violations
issued in inspection Report 50-336/91-05. These violations involved the failure to follow
i
procedures by station personnel involving the installation of an environmentally qualified
electrical splice and were the subject of an enforcement conference on March 27,1991. We

found your corrective actions in this matter comprehensive and satisfactory
