Response to Examination Report OL-93-01
Dear Mr. Martin:
On June 14, 1993, the Nuclear Regulatory Commission (NRC) issued Examination Report OL-93-01 regarding licensed operator examinations conducted at Davis-Besse Nuclear Power Station (DBNPS) during the week of May 24, 1993.
The Examination Report discussed concerns regarding Jiolations of the examination security agreement and requested that Toledo Edison provide a written response identifying the corrective actions taken to prevent recurrence of this event.
During the preparation for adminirtration of the May 24 - 26, 1993, licensed operator examination, three violations of the security agreement occurred. These violations involvrd three separate training instructors that had signed the security agreement and subsequently taught classes to or evaluated the requalification candidater The security agreement requires that individuals that acquire specialized knowledge about NRC license or requalification examinations be restricted from instruction of examination candidates.
Contrary to this agreement the following <iolations, involving three individuals I
that had signed the security agreement, occurred:

On May 18,-1993, an instructor evaluated a crev vhich contained a
[
requalification candidate.
At that time, the instructor had no knowledge of the examination content.

On May 18, 1993, a third individual, from the Operations f
Department, taught a class on upcoming procedure changes.
A

student in this class was a requalification candidate. This i
j individual did have know. ledge of the examination content.
In each case, the individuals involved failed to take proper i
precautions, after signing the security agreement, to ensure that no i
breach of the agreement could occur. The classroom participants should j
i have been revieved for exam candidates prior to administering the class.
In addition, administrative controls were not in place to prohibit training supervisors from assigning instructors to conduct i
training involving exam candidates.

i In the three cases above, Toledo Edison and the NRC examiner reviewed the circumstances surrounding the events and determined that no i
examination compromise occurred.

Corrective Actions to Prevent Recurrence:
l Toledo Edison has taken the following actions to prevent recurrence of l
a breach of security agreement:
Operations Training individuals involved in the incident vere l
counseled concerning their failure to properly adhere to the l
l requirements of the examination security agreement. A " Memorandum of j
Inappropriate Performance" was vritten to document the individual's l
involvement in this incident and has been placed in their personnel i
l files within Nuclear Training.
To heighten personnel awareness of the significance of these events and to ensure an understanding of the commensurate responsibilities associated vith an individual's signature, the issue was discussed at an Operations Training staff meeting on May 21, 1993. Those not in attendance for the staff meeting were individually counseled.
Additionally, the minutes from this staf f meeting vere reviewed by each Operations Training Unit instructor.
j The security agreement used at Davis-Besse was revised to include the i
following:
l 1.
The security agreement now lists the candidates scheduled for the initial or requalification examination.
2.
The revised security agreement prohibits instructors from instruction in any training program without specific permission of the Supervisor - Nuclear Operations Training.

The term " instruction" has been defined to include as tutoring, examination or evaluation conducted prior to the examination.
