SUBJECT: NRC INSPECTION REPORT NO. 50-213/93-16

This letter transmits the report of the resident safety inspection conducted by
,
i
Messrs. W. Raymond and P. Habighorst of this office during the period July 25,1993 to
August 28,1993, at the Haddam Neck Plant, Haddam Neck, Connecticut. The report covers
l
activities autho'ized by NRC License No. DPR-61. This inspection was focused on issues
I
important to public health and safety. Preliminary findings were discussed with Mr. J. Stetz
of your staff at the conclusion of the inspection.
During this report period, we found Haddam Neck was operated in a safe and conservative
,
l
manner. Plant personnel performed very well to complete startup from the outage, and
'
resume power operations. Actions to investigate an anomalous quadrant power tilt were
j
timely and expertly done. The response by the plant staff showed a good regard for safety
'
and sensitivity to core performance. Support by plant engineering to investigate the anomaly
in a timely manner was excellent.
The enclosed inspection report provides our assessment of several outage performance issues
and events. Overall, we determined that plant personnel responded very well to significant
,
challenges and emergent issues during the outage. Some of the events occurred during
implementation ofimproved test procedures, resulting in the identification of equipment or
design problems. We encourage your initiatives to improve test methods and to uncover
dtSciencies. However, our assessment of the events does show the need for continued
management attention to assure the adequacy of procedure reviews, self-checking by plant
personnel of their actions during the conduct of routine duties, that schedule pressures do not
impact performance, and that preparations are completed well in advance of the outage.
