SUBJECT:

ROUTINE RADIATION PR0fECTION INSPECTION AT DUANE ARNOLD ENERGY

CENTER

This refers to the routine safety inspection conducted by Mr. T. J. Kozak of

this office on August 9 - 13, 1993. The inspection included a review of

authorized activities for your Duane Arnold Energy Center. At the conclusion

of the inspection, the findings were discussed with those members of your

staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of a selective examination of procedures

and representative records, interviews with personnel, and observations of

activities in progress.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The violation is of concern because it is the third time

in the past several months that a high radiation barrier has been crossed

without proper authorization. Although each event appeared to have a

,

different root cause, the frequency of the unauthorized entries into these

areas is an indication that your controls have not been effective. Additional

management focus appears' warranted in this area. Also during this inspection,

I

tours of radiologically controlled areas identified problems with

housekeeping. As a result of these observations, it appears additional

management attention is warranted to ensure that contaminated area

housekeeping improvements are made.

You are required to respond to this letter and should follow the instructions

'

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.
