In the second paragraph on page 1 of the subject report, it is stated that the Nuclear Energy Services (NES) carried out the decommissioning activities in two phases. This statement is incorrect. Although the decommissioning activities were conducted in two phases, NES was involved only in the secono phase of the project. Phase I of the decommissioning which included removal of the reactor core moderator, graphite thermal column, fuel boxes, most of the concrete shield blocks, etc., was carried out internally by UCLA.
