1.0 OICANIZATION Provide an organization chart identifying each organizational elemnt that functions under the QA Program.
I Identify the individuals responsible for training and certification, documnt control, and auditing.

The individual responsible for training and certification is Mr. Howard Danahue, Radiation Safety Officer at Electric Boat's Groton, Connecticut and Quonset Ibint, Rhode Island facilities. The individual responsible for auditing and document control is Mr E. L. Zubritsky.
2.0 QUALITY ASSURA!CR M40 GRAM State that training, prior to engagemnt, for all QA functions is required according to written procedures. QA Program revisions will be made according to written procedures and with marmgement approval.

Before an individaal is allowed to perform any QA functions, he must be trained as a radiographer and to the requiremnts of Electric Boat Division's Receipt, Shirrent and Transport procedures as approved by managemnt and included in Electric Boat Division's Radiography License.
3.1 JOCil9NT 00lTTROL State that all documnt changes will be performd according to written procedures approved by managemnt.
