SUBJECT: NOTICE OF VIOLATION
This refers to the inspection conducted by Troy W. Pruett of this office on
May 22 and August 12, 1993. The inspection included a review of activities
authorized for your Anchorage, Alaska facility. The findings were discussed
with you at the conclusion of the inspection.
The inspection was an examination of the activities conducted under your NRC '
license as they relate to radiation safety and compliance with the
Commission's rules and regulations and the conditions of your license. Within
these areas, the inspection consisted of selective examinations.of procedures
and representative records, interviews with personnel, and observation of
.
activities in progress. The purpose of the inspection was to determine
,
whether activities authorized by the license were conducted safely and in
i
accordance with NRC requirements.
Based on the results of this inspection, certain of your acti.vities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violations are of concern because they demonstrate a
,
breakdown in the oversight of the radiation safety program. Additionally,
three of the violations, the failure to perform leak tests, the failure to
maintain exposure records, and the failure to maintain inventory records, were
l
repeated from the last inspection performed on July 18, 1991.
In your
response you should describe the specific actions you have taken and any
additional actions you plan to develop a tracking and record retention system
to prevent recurrence of these repetitive violations. Additionally, you need
to submit a license amendment to NRC Region V, which reflects your current
business address.
You are required to respond to this letter and should follow the instructions
i
specified in the enclosed Notice when preparing your response. After
reviewing your response to this Notice, including your proposed corrective
actions and the results of future inspections, the NRC will determine whether
further NRC enforcement action is necessary to ensure compliance with NRC
regulatory requirements. The third occurrence of a violation could result in
a civil penalty or suspension of licensed activities.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
