This is in regards to your letter dated September 15, 1993, directed to Mr. Kris Kumar of Power Inspection, Inc. providing notification that our Quality Assurance (QA) Program Approval for Radioactive Material Packages, Approval No. 0654, Rev. O, expired on June 30, 1993.
Whereas our QA Program originally submitted on May 16, 1988, has not changed, please reinstate our QA Program and reinstate Power Inspection, Inc. as a user of Certificate of Compliance No. 9033.
Enclosed with this letter is the $370.00 reinstatement fee.
