NRC-38-92-287 Modification No. 3 Page 2 of 5 The purpose of this modification is to incorporate changes to the Statement of Work for clarification purposes, at no additional cost or obligation to the Government. Accordingly, the following changes are hereby made:
1.
Under Section C.1, Statement of Work, Paragraph (b), add the following sentence.
"The Contractor shall provide with each dosimetry shipment an invoice that includes the serial number of dosimeter shipped (e.g., the IR'4 number), and the dosimeter name."
2.
Under Section C.1, Statement of Work, Paragraph (c), is deleted in its entirety and the following is substituted in lieu thereof:
"The Contractor shall deliver dosimeters at the request of the authorized individuals listed below.

NRC Region I, ATTN: Joseph Furia (RS0), 47., Allendale Road, King of Prussia, PA 19406.
b.
NRC Region II, ATTN: Bill Cline (RS0), 101 Marietta Street, Suite 2900, Atlanta, GA 30323.

Under Section C.1, Statement of Work, Paragraph (e), add the following:
j "The Contractor shall provide a 3.5 inch, high density diskette. The i
Contractor shall certify by written documentation that the diskette and the Dosimetry report (reflected under Par. F.5) are identical in content."
4.
Under Section E - Inspection and Acceptance, subparagraph E.2, Place of Inspection and Acceptance, is deleted in its entirety and the following is substituted in lieu thereof:
" Inspection and acceptance of dosimeter shipments shall be made by contacts at the destinations listed in Paragraph C.1 (c).
Inspection and acceptance of the written dosimetry reports shall be made by the 1
Radiation Safety Officers at the destinations listed in Paragraph F.5.
1 Inspection and acceptance of the diskette dosimetry reports shall be made by the Project Officer at the destination listed in Paragraph G.I."

Under Section F - Deliveries or Performance, subsection F.3, Place Of, Delivery add the following:
"(c) The following items shall be delivered, with all charges paid by

the Contractor; (1)
Dosimeter shipments shall be delivered to the destinations specified in Paragraph C.1 (c) and other destination as j
requested by authorized individuals.
(2)
Written dosimetry reports shall be delivered to the Radiation Safety Officers at the destinations listed in i
i Paragraph F.5 with a copy to the Project Officer listed in Paragraph G.I.
(3)
Diskette dosimetry reports shall be delivered to the Project Officer at the destination listed in Paragraph G.I."

Under Section F - Deliveries or Performance, subsection F.5 Dosimetry Reports is deleted in its entirety and the following is substituted in i
lieu thereof:
"The Contractor shall provide a written report of dosimetry results to each Regional and Headquarters Radiation Safety Officer (RS0) listed below within 30 days of received the returned dosimeters. Any record for a Headquarters employee in an office other than HMSS, or AE00, shall be reported on the NRR report. Each report shall contain only the information for the employees in that office or region. The list of.
i employees in each office and region is subject to change. Reports shall contain report date, employee's name, social security number, period monitored, and exposure results, including current, quarterly, year-to-

The Contractor shall provide an annual summary report to RS0's for each calendar year in two formats; first, one page per employee, and second, fu,11 account in the same format as the quarterly report. The Contractor i
shall make corrections to data on quarterly reports upon written request of the RS0s, i.e., badge may inadvertently receive a radiation dose that employee did not; provide an immediate report by telephone to the appropriate RS0s within two (2) working days of any dosimeter reading in excess of 400 millirem; provide review and explanation of dosimetry results and anomalies by making technical personnel available to NRC by telephone.
The Contractor shall verify that each report of dosimetry results does not contain any records provided on previous reports. Revised records may be retransmitted to correct erroneous information if NRC is informed and concurs. Any questionable data shall be resolved with the appropriate RSO prior to issuing the quarterly or annual report.
The Contractor shall verify that each report of dosimetry results accurately reflects all personnel issued a dosimeter during the monitoring period according to NRC records returned with the dosimeters.
Each report shall' accurately note all dosimeters received late and all dosimeters not returned.
The Contractor shall provide the assigned wear period for badges returned late rather than the end date for the period in which the, dosimeter was actually returned.
The Contractor shall assure that NRC is not billed for badges that were'

sent to NRC in error and were not used.
