Beaver Valley Power Station, Unit No. 1 Docket No. 50-334, License No. DPR-66 Medical Status of Licensed Operator (10 CFR 50.74/10 CFR 55.25)
The purpose of this letter is to notify you of a change in medical status of a
licensed operator in accordance with the requirements of 10 CFR 50.74/10 CFR 55.25.
Mr.
L.
G.
Schad (Docket No. 55-5180, License No. SOP-2516-6) has been determined to have vision which is outside of the acceptable limits.
He was examined by Dr.
G.
Eernich (OS-006745-L) of Duquesne Light Company's Nuclear Power Division Medical Section and has been required to wear corrective lenses while performing licensed duties.
This change in status was determined during a physical examination on July 29,
- 1991, and was not reported within the 30 day time requirement.
This omission of notification was discovered when Mr.
Schad's license renewal application was submitted requiring corrective lenses and NRC Regional review questioned the changes without prior formal notification.
This is not in accordance with 10 CFR 50.74 rotification requirements.
Duquesne Light Company has taken the appropriate steps to ensure there are no other medical status changes that have not been reported as required and has instituted additional requirements to preclude any further recurrence.
This information is included as an attachment to this notification.

Upon notification of this deviation in medical status, Mr. Schad was interviewed via telephone to ensure he was aware of his i
corrective lens restriction.
2)
Region I
was notified that Mr. Schad does have a corrective lens restriction on his license and that Duquesne Light Company did not report this within 30 days as required by 10 CFR 50.74.
3)
An internal audit was performed to ensure that there were no other required notifications for chenge of medical status that were not performed as required.
No other discrepancies exist.

A meeting was conducted between the Director-Operations Training and the Director-Medical Section to re-enforce the need for a determination by the physician of a change in medical status and for that status change to be reported to the Director-Operations Training for notification to the NRC as required.
5)
Additional checks of completed Report of Facility Operator's.
Medical Exam (summary of physician's results of medical exam)
I will be completed by the Director-Operations Training and the Operations Training Section clerk.
This check will include the reverification of any status change.
