As of this date I have not received my renewal package for my QA Program Recertification.
I am enclosing a copy of our Quality Assurance Program for use, maintenance and repair of shipping containers for radioactive material, for your review and acceptance.
Since I haven't received the proper forms, I feel this notification is elligible for timely renewal.
Enclosed is my check in the amount of $280.00 to cover the renewal fees as stated in the July 14, 1993 bulletin page 124.
