SUBIECT: EXAMINATION REPORT NOS. 50-352/93-18 and 50-353/93-18
During the week of July 19,1993, the NRC administered initial examinations to employees
of your company who had applied for licenses to operate the Limerick Unit 1 & 2 facilities.
The evaluation addressed areas important to public health and safety and were conducted in
accordance with Revision 7 (1/1/93) to NUREG-1021, " Operator Licensing Examiner
Standards." The purpose of the evaluation is the examination of the individual operators to
assure safe plant operation. At the conclusion of the examinations, the preliminary findings
were discussed with those members of your staff identified in the enclosed report.
All applicants passed the examination and were generally well prepared for the examinations.
One SRO upgrade candidate also passed a retake of the operating examination section C.
Your staff is commended for their cooperation during the examination preparation and
administration. Section 3.0 of the enclosed report contains details of the applicants
performance and the NRC examiners' observations.
As a result of this NRC examination, we identified a weakness in operator performance in
the identification and proper action related to core thermal hydraulic (T-H) instabilities
(section 3.5, Details). The weak performance was inadvertant manual scrams because of
misidentification or improper action related to symptoms of pressure control system
oscillations. The operator applicants confused these symptoms for T-H instabilities. The
examiners noted that the identification of core T-H instabilities and the adequacy of licensee
procedure guidance were collectively cited as a deviation from licensee written commitments
during last year's NRC staff examination (Report No. 50-352,353/92-21). Your
organization responded to the deviation by continuing to support the guidance as the latest
from the vendor group. At the time of last year's cramination, no examples were noted on
how the weak guidance adversely affected performance. This year, the weak performance
noted above appears to be due to the weak guidance and may be due to poor training in

this area. This leads to a question on how operators will actually perform in the plant when
faced with normal versus abnormal core power oscillations. At the exit meeting when this
question was discussed, your staffindicated they would pursue a resolution. Accordingly,
we request that you inform us within 30 days of the date of this letter what actions you took
or plan to take in this area.
Further, we rcceived yot.t letter dated June 28,1993, in response to our letter of
May 27,1993. Thank you for informing us of the corrective and preventive actic,ns
documented in your letter related to the proper notification to the NRC for an operator
license termination. The enclosed report reviewed your corrective actions.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and
its enclosure will be placed in the NRC Public Document Room.
