Enclosed is Appendix "F" of our Radiation Safety Manual which covers our Quality Assurance Program for " Transporting Packages of Radioactive Materials."

This section includes an organizational chart listing responsibilities by position for administration of this program.

We have also enclosed Appendix "E" which lists the qualifications of the key personnel administrating the program.

It is our understanding that the amendment fee for making the requested modification to our license is $320.00 and a check in that amount is enclosed.

l NOTICE The management of MMR Group, Inc., establishes and implements this Quality Assurance Program for Transporting Packages of Radioactive Material. Training for all QA Program functions, prior to engagement in these functions, is required. QA Program grevisions shall be made ac. cording to written procedures with management approval.

Design and fabrication of packages for shipping radioactive materials shall not be part of this QA Program.

The organizational responsibilities at MMR Group, Inc. for administration of this program are presented in the chart below:
