NOTICE OF VIOLATION

NRC INSPECTION REPORT NO. 50-245/93-20

'

This refers to the announced inspection conducted by Mr. Leonard Cheung of this office on

July 19-23,1993. The inspection included a review of activities authorized for your

Millstone Unit I facility. At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the enclosed report. Additionally, the status

,

of the inspection findings was discussed with Mr. R. Ewing during a subsequent telephone

conversation on August 5,1993, as a result of the staff review of additional documents

-

transmitted to the NRC on July 28 and July 30,1993.

.

The inspection was directed toward areas important to public health and safety. Areas

examined during this inspection included the resolution and corrective actions taken for open

,

items identified previously during the Electrical Distribution System Functional Inspection

,

'

(EDSFI). Within this scope, the inspection consisted of selective examinations of design

documents, procedures and representative records, interviews with personnel, and

observations by the inspector.

.

Based upon the results of this inspection, one of your activities appeared to be in violation of

NRC requirements, as discussed in the enclosed Notice of Violation (Notir ). The violation

is of concern because your corrective actions for an identical problem (fa. are to complete

biennial review of station procedures identified in an NRC issued violation almost two years

ago) did not prevent it from recurring. Further, we found that the number of unreviewed

'

procedures actually increased during the past two years. You are required to respond to this

letter and you should follow the instructions of the enclosed Notice when preparing your

'

'

response. In your response, you should document the specific actions taken and any

additional actions you plan to prevent recurrence. In particular, include in your response

why a previous NRC violation went uncorrected. After reviewing your response to this

l

Notice, including your proposed corrective actions and the results of future inspections, the

NRC will determine whether further .NRC enforcement action is necessary to ensure

'

compliance with NRC regulatory requirements.

actions for four previously identified issues were found to be adequate and the items are

closed. The corrective actions for the other four items were incomplete. Therefore, these
